摘　　要：目的:探讨血清因子与免疫因子对慢性阻塞性肺疾病患者肺功能与病情的影响。方法:选择2016年1月—2017年12月间于我院就诊的COPD患者117例,根据患者病情分为A组(61例急性加重期)和B组(56例稳定期)。同时选取同期在我院体检的健康志愿者80例作为对照组。检测比较各组的白细胞介素-6(IL-6)、IL-8、C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)等血清炎症因子以及淀粉样蛋白A(SAA)和干扰素G诱导蛋白10(IP-10)等免疫因子水平,同时测定各组的肺功能以及CAT评分。结果:A组患者血清因子和免疫因子水平均明显高于B组和对照组,差异具有统计学意义。A组患者的FEV 1和FEV 1/FVC明显低于B组和对照组,A组患者的CAT评分明显高于B组,差异具有统计学意义。COPD患者的肺功能与血清IL-8、IL-6、TNF-α、CRP、SAA以及IP-10水平呈负相关,CAT评分与其呈正相关。结论:COPD患者血清中的炎性因子以及免疫因子水平随病情变化发生动态改变,含量明显升高,且与患者的肺功能呈负相关。Objective:To discuss the effect of serum and immune factors on lung function and pathogenetic condition in patients with chronic obstructive pulmonary disease.Methods:117 COPD patients were selected in our hospital from January 2016 to December 2017.They were divided into A group(61 patients with acute exacerbation COPD)and B group(56 patients with stable stage)according to the pathogenetic condition.80 healthy cases were selected as the control group.The levels of IL-6,IL-8,CRP,TNF-α,SAA and IP-10 were compared.The lung function and CAT scores were compared.Results:The levels of serum and immune factors in A group were significantly higher than those in B group and control group.The FEV 1 and FEV 1/FVC in A group were significantly lower than those in B group and control group.The CAT score in A group was significantly higher than that in B group.The FEV 1 and FEV 1/FVC were negatively correlated with IL-6,IL-8,CRP,TNF-α,SAA and IP-10.CAT score was positively correlated with them.Conclusion:The levels of serum and immune factors in patients with COPD change dynamically with the severity of pathogenetic condition.The levels increase significantly and negatively correlated with the lung function of the patients.
