阴道侧壁固定术辅助经阴道全子宫切除术治疗Ⅲ度子宫脱垂合并阴道膨出患者的疗效及安全性分析

摘　　要：目的:探讨阴道侧壁固定术辅助经阴道全子宫切除术(TVH)在Ⅲ度子宫脱垂合并阴道膨出患者中的应用效果。方法:选取我院Ⅲ度子宫脱垂合并阴道膨出患者82例(2017年5月~2019年12月),依据随机数字表法分组,各41例。对照组采用阴道壁修补术辅助TVH术,观察组采用阴道侧壁固定术辅助TVH术。比较两组疗效、术中出血量、手术时间、排气时间、住院时间、术前、术后1 d、3 d、7 d视觉模拟量表(VAS)评分、术后并发症发生率。结果:观察组治疗总有效率97.56%(40/41)高于对照组78.05%(32/41),术后并发症发生率4.88%(2/41)低于对照组21.95%(9/41)(P<0.05);两组术中出血量、手术时间、排气时间、住院时间比较无明显差异(P>0.05);术后1 d、3 d、7 d,观察组VAS评分低于对照组(P<0.05)。结论:阴道侧壁固定术辅助TVH术治疗Ⅲ度子宫脱垂合并阴道膨出患者疗效显著,术中出血量少,手术时间、排气时间、住院时间短,可减轻术后疼痛,降低术后并发症发生率。Objective:To investigate the effect of vaginal lateral wall fixation assisted transvaginal hysterectomy(TVH)in patients with third-degree uterine prolapse and vaginal bulging.Methods:Eighty-two patients with third-degree uterine prolapse and vaginal bulging in our hospital(May 2017-December 2019)were selected,and grouped according to random number table method,41 cases each.The control group used vaginal wall repair assisted TVH operation,and the observation group used vaginal wall fixation assisted TVH operation.The efficacy,intraoperative blood loss,operation time,exhaust time,hospitalization time,visual analog scale(VAS)scores 1 day,3 days and 7 days preoperative and postoperative and the incidence of postoperative complications were compared between the two groups.Results:The total effective rate of treatment in the observation group was 97.56%(40/41)higher than that in the control group 78.05%(32/17),and the postoperative complication rate was 4.88%(2/41)lower than that in the control group 21.95%(9/41,P<0.05);there was no significant difference between the two groups in intraoperative blood loss,operation time,exhaust time,and hospital stay(P>0.05);at 1,3,and 7 days after surgery,the VAS score of the observation group was lower than that of the control group(P<0.05).Conclusion:Vaginal lateral wall fixation assisted TVH surgery is effective in the treatment of patients with third-degree uterine prolapse and vaginal bulging,with less intraoperative bleeding,short operation time,exhaust time,and hospital stay,which can reduce postoperative pain and reduce the incidence of postoperative complications.
