血必净注射液联合美罗培南治疗老年重症肺炎的临床效果评价及其对血清炎症因子的影响

摘　　要：目的:探讨血必净注射液联合美罗培南治疗老年重症肺炎的效果及其对重症肺炎患者血清炎症因子的影响。方法:选取本院2018年6月~2019年10月收治的老年重症肺炎患者96例为研究对象,按随机数字表法分为对照组和观察组,每组48例。对照组静脉注射美罗培南治疗,观察组给予美罗培南+血必净注射液静脉注射治疗,观察两组的疗效、退热时间、肺鸣音消失时间、咳嗽消失时间和住院时间,治疗前后炎症因子、血气指标的变化等。结果:治疗后,观察组总有效率高于对照组(P <0. 05);观察组退热时间、肺鸣音消失时间、咳嗽消失时间和住院时间均短于对照组(P <0. 05);观察组氧分压(PaO2)、血氧饱和度(SpO2)、氧合指数(PaO2/Fi O2)水平高于对照组,二氧化碳分压(PaCO2)和血清白细胞介素(IL-6)、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)、C-反应蛋白(CRP)水平低于对照组,差异均有统计学意义(P <0. 05)。结论:血必净注射液与美罗培南联用治疗老年重症肺炎能有效降低机体炎症反应程度,改善动脉血气指标和缩短住院时间,提高临床疗效。Objective:To investigate the curative efficacy of Xuebijing injection combined with meropenem in treatment of elderly severe pneumonia and effects on serum inflammatory cytokines.Methods:96 elderly patients with severe pneumonia admitted to our hospital from June 2018 to October 2019 were selected and randomly divided into control group and observation group,with 48 cases each.The control group was given intravenous drip of meropenem,and the observation group was given additional Xuebijing injection on the basis of control group.The clinical efficacy,disappearance of fever time,disappearance of lung wheezing sound time,disappearance of cough time,hospitalization time,and the change of blood gas index and inflammatory cytokines level before and after treatment were analyzed.Results:The clinical efficacy of the observation group was better than that of the control group(P<0.05);the disappearance time of fever,lung wheezing sound and cough,as well as hospitalization time were significantly shorter than those of control group(P<0.05);After treatment,the level of PaO 2,SpO 2,PaO 2/FiO 2 in the observation group were significantly higher than those in the control group(P<0.05);PaCO 2 and serum IL-6,TNF-α,PCT,CRP levels of treatment group were significantly lower than those in control group(P<0.05).Conclusion:Xuebijing injection combined with meropenem has good clinical efficacy in the treatment of elderly severe pneumonia patients by reducing the degree of inflammatory reactions,improving arterial blood gas indexes and shortening hospitalization time,and thus improving clinical efficacy.
