陈永灿调升降畅气血辨治老年杂病的临证经验

摘　　要：文章介绍了陈永灿主任中医师运用调升降、畅气血的治疗大法辨治老年杂病的临证经验,并附医案三则。陈永灿认为,老年人所患疾病可分成两大类,一是老年基础疾病,如高血压病、糖尿病、脑血管病等慢性疾病;二是老年多发病,如骨质疏松、前列腺增生、皮肤瘙痒症等。陈永灿将老年基础疾病合并老年多发病称为老年杂病,认为其根本病机为升降失司,气血不畅。治疗上,提出调升降、畅气血的治疗大法。处方用药上,喜用简易名方化裁组方,善施花类药物宣畅气血。如魏氏补脑汤合升降散、清震汤治疗老年抑郁症,自拟"三花百草饮"合封髓丹治疗老年瘙痒症,升陷汤、四君子汤合当归补血汤治疗老年汗出症等,取效良好。This paper introduces the clinical experience of CHEN Yong-can,the director of Chinese medicine doctor,in the treatment of miscellaneous diseases in the elderly with the methods of regulating and elevating and clearing up qi and blood,and three medical cases are attached.CHEN believes that diseases of the elderly can be divided into two categories.First is basic diseases of the elderly,such as hypertension,diabetes,cerebrovascular disease and other chronic diseases;second is the elderly frequently-occurring disease,such as osteoporosis,prostatic hyperplasia,pruritus,and so on.CHEN called the basic diseases of old age combined with the frequent senile diseases as miscellaneous diseases of old age,and believed that the basic pathogenesis was the loss of division,qi and blood obstruction.In terms of treatment,the therapeutic methods of regulating elevation and clearing qi and blood were put forward.On prescription medication,he likes to use simple prescriptions,and he is good at applying flower medicines to elevating qi and blood.For example,Weishi Bunao Decoction,Shengjiang Decoction and Qingzhen Decoction in the treatment of senile depression,self-made Sanhua Baicao Decoction combined with Fengsui Pill in the treatment of senile pruritus,Shengxian Decoction,Sijunzi Decoction and Danggui Buxue Decoction are used to treat senile hyperhidrosis,have achieved good results.
