火针刺络联合针刺引流治疗肢体淋巴水肿的临床研究

摘　　要：目的:观察针刺引流法配合综合消肿疗法治疗肢体淋巴水肿的临床疗效。方法:将65例肢体淋巴水肿患者分为观察组33例、对照组32例。观察组采用火针刺络联合针刺引流、配合综合消肿疗法(包括手法淋巴引流按摩、肢体运动锻炼、皮肤护理、抬高患肢);对照组仅采用综合消肿疗法;针刺1次/周,综合消肿疗法5次/周,疗程8周。观察两组临床水肿减轻的疗效,以及疼痛、酸胀、乏力等症状的改善情况。结果:观察组治疗有效率为93.9%(31/33),高于对照组87.5%(28/32)(P<0.05);与治疗前比较,两组患者治疗后患肢周径均缩小(P<0.05,P<0.01),且观察组优于对照组(P<0.05);观察组疼痛感、酸胀感、乏力感症状减轻优于对照组(P<0.05);彩超可见软组织层水肿改善;两组均未见明显不良反应。结论:火针刺络联合针刺引流治疗肢体淋巴水肿有效安全。Objective:To evaluate the efficacy of the treatment of hot-needle pricking combined with acupuncture drainage on limb lymphedema.Methods:A total of 65 cases were collected and 33 cases were assigned as the treatment group and 32 as the control group.Both groups were given regular comprehensive detumescence therapy(manual lymphatic drainage,physical exercise,skin care and raise the affected limb were including).An additional treatment of the hot-needle pricking combined with acupuncture drainage were given to the treatment group.The course of treatment for both groups was 8 weeks,during which comprehensive detumescence therapy were taken 5 times per week,and acupuncture 1 time per week.The efficacy were examined by the degree of lymphedema,levels of pain,soreness and fatigue.Results:The total effective rate was 93.9%(31/33)in the treatment group,higher than that of the control group(87.5%,28/32)(P<0.05).Compared with before treatment in the same group,circumference of limbs were remarkably decreased in both groups after treatment(P<0.05,P<0.01),and treatment group was better than the control group(P<0.05).The levels of pain,soreness and fatigue were remarkably less in the treatment group than in the control group(P<0.05).The thickness of soft tissue layer was reduced measured by ultrasound.No obvious adverse reactions occurred in both groups.Conclusion:It is effective and safe to take hot-needle pricking combined with acupuncture drainage in the treatment of limb lymphedema.
