清热消癥法治疗中期糖尿病肾病临床疗效观察

摘　　要：目的:观察清热消癥法对中期糖尿病肾病的治疗效果。方法:将2016年6月至2018年8月就诊于北京中医药大学东直门医院的40例中期糖尿病肾病患者分为对照组20例,治疗组20例。对照组予以西医基础治疗,治疗组在西医基础治疗的基础上予清热消癥法治疗。治疗周期为24周。分别在第0、4、12、24周对患者进行随访观察,观察指标包括患者一般情况、24h尿蛋白定量(24hUTP)、肾功能、肝功能、中医证候变化等。结果:治疗组在第4、12周时与治疗前比较,24hUTP显著下降(P<0.01,P<0.05)。对照组在治疗第12、24周,24hUTP较治疗前显著升高(P<0.05)。治疗第24周时,治疗组中医证候评分与治疗前比较,显著下降(P<0.05)。糖脂代谢、血常规及肝功能治疗前后均无明显差异。结论:西医基础治疗结合清热消癥法治疗中期糖尿病肾病较单纯西医治疗在降低尿蛋白及改善中医证候评分方面更具有优势。Objective:To observe the therapeutic effect of clearing heat and dispersing mass therapy on middle stage of diabetic kidney disease.Methods:Forty patients with middle stage diabetic nephropathy in Dongzhimen Hospital,Beijing University of Traditional Chinese Medicine from June 2016 to August 2018 were divided into control group(20 cases)and treatment group(20 cases).The control group was treated with basic western medicine.For the treatment group,besides the basic treatment of western medicine,the method of clearing heat and dispersing mass therapy was used.The treatment course was 24 weeks.Patients were followed up at week 0,4,12 and 24 to observe TCM syndromes and laboratory indicators.The observation indexes included general condition of patients,24-hour urine total protein quantity(24 hUTP),renal function,liver function,TCM syndrome changes,etc.Results:In the treatment group,24 hUTP was significantly decreased at week 4,12 compared with that before treatment,and the difference was statistically significant(P<0.01,P<0.05).In the control group,24 hUTP was significantly higher at week 12 and 24 than before treatment,and the difference was statistically significant(P<0.05).At week 24,the TCM syndromes score of patients in the treatment group was significantly lower than that before treatment(P<0.05).There were no significant differences in glycolipid metabolism,liver function and blood routine before and after treatment in the treatment group,and no statistically significant differences compared with the control group.Conclusion:Compared with western medicine alone,western medicine combined with clearing heat and dispersing mass therapy has more advantages in reducing urine protein and improving TCM syndrome score.
