胃舒颗粒联合耳穴压豆治疗肝郁脾虚型十二指肠溃疡的临床研究

摘　　要：目的:观察胃舒颗粒联合耳穴压豆治疗肝郁脾虚型十二指肠溃疡(DU)的有效性。方法:选取2018年1月至2019年5月就诊于山东中医药大学附属医院的肝郁脾虚证DU患者(活动期)50例,按随机对照原则分为试验组和对照组,各25例。幽门螺杆菌(Hp)(+)者均给予铋剂四联根除疗法。对照组给予泮托拉唑40mg,口服,2次/d,连续6周;试验组在此基础上,给予自制剂胃舒颗粒,3g/次,口服,3次/d,联合耳穴压豆(肝、脾、胃、交感、十二指肠)疗法。两组治疗4周后评价中医症状积分、证候总积分、胃镜疗效;治疗6周后,检测血清胃泌素(GAS)、胃蛋白酶原(PG)-Ⅰ、PG-Ⅱ。Hp(+)者停药1、6个月后复查Hp,评价其根除及复发情况。结果:治疗后两组症状积分比较,除胃脘胀痛、反酸嘈杂外,试验组其余症状改善、总积分改善、显效率、总有效率均优于对照组(P<0.05,P<0.01);两组胃镜疗效均有较高的显效率及总有效率,组间比较,差异无统计学意义;两组Hp根除比较,均呈现Hp高根除率、低复发率特点,组间比较无显著性差异;血清GAS、PG-Ⅰ、PG-Ⅱ比较,试验组作用显著优于对照组(P<0.05,P<0.01)。结论:胃舒颗粒联合耳穴压豆治疗肝郁脾虚型DU可明显改善患者的症状及相关理化指标。Objective:To observe the efficacy of Weishu Granules combined with ear acupoint pressing beans in treating duodenal ulcer(DU)with stagnation of liver depression and spleen deficiency.Methods:A total of 50 DU patients with stagnation of liver depression and spleen deficiency syndrome(active period)in Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2018 to May 2019 were selected and randomly divided into two groups(test group,control group)according to the randomized control principle with 25 cases in each group.Patients in both groups of Hp(+)were given bismuth quadruple eradication therapy.Control group:Pantoprazole 40 mg was given for 6 weeks,po.Bid;test group:on this basis,selfprepared Weishu Granules,3 g po.tid,combined ear point pressure beans(liver,spleen,stomach,sympathetic,duodenal)therapy.After 4 weeks of treatment,the two groups were evaluated by TCM syndromes,gastroscope.After 6 weeks of treatment,gastrin(GAS),pepsinogen(PG)-I,and PG-II were test.Patients with Hp(+)require Hp test to evaluate eradication and recurrence one month after medication and 6 months after treatment,respectively.Results:After the treatment,the symptom scores of the two groups were compared,except that there was no significant difference between stomachache and anti-acid noise,all the residual symptoms of the test group were improved,the total score,the apparent efficiency and the total effective rate were better than those of the control group(P<0.05,P<0.01).There was no difference between the two groups on gastroscopes.The two groups showed Hp high eradication rate and low recurrence rate,and there was no significant difference between the groups.Compared with GAS,PG-I and PG-II,the test group was significantly better than the control group(P<0.05,P<0.01).Conclusion:Weishu Granules combined with ear acupoint pressing beans in treating DU with stagnation of liver depression and spleen deficiency can significantly improve the symptoms and related physical and chemical indexes.
