体现辨证论治干预效果的证候疗效特色结局指标集初探研究

摘　　要：根据证候疗效应回归其本来定位的观点——证候疗效是指通过辨证论治、针对证候进行疾病防治所产生的效果,是从干预措施的角度对临床结局及其评价指标的一种分类,突出干预措施的治疗特点,可以通过构建证候疗效特色结局指标集来体现辨证论治的效果特点。体现辨证论治干预效果的结局指标集合具有整体性、个体化、动态化的特点。由于辨证论治产生的效果可以根据其治疗的不同阶段进行分层表达,在整体层次的效果可以用患者结局报告及中医医生结局报告来评价。对于微观层次或疾病层次的效果则可以借鉴目前已经被广泛使用的指标与方法来评价。两个层次的评价指标和方法,相互联系但不可替代,具体选择哪一层次的方法与指标则要根据临床实践或研究目的来确定。It is known that TCM syndromes effect should be reset on its original position,it meant that effectiveness of TCM syndromes referred to the effectiveness of TCM syndromes differentiation or the application of TCM syndromes for treating patients,in other words,to the classification of outcomes and measurements in the view of intervention.The methods for establishment of intervention-related specific outcome sets(In-SOS)helped to highlight the effectiveness characteristics of TCM syndromes differentiation in clinic.The new model of TCM-related specific outcome sets(TCM-SOS)stood for the characteristics of wholeness,individuality and dynamic of TCM syndromes differentiation.Since the effect of treatment based on TCM syndromes differentiation can be expressed in different levels according to different stages,the effect at the overall level can be evaluated by the patient reported outcome(PRO)and the TCM clinician reported outcome(CRO).The effect of microcosmic or disease levels can be evaluated by referring to the outcomes or measurements that have been widely used now.The both evaluation measurements were interrelated but irreplaceable.The specific selection of these outcomes or measurements should be determined according to clinical practice or research’s purpose.
