朱宗元基于“谨守病机,异病同治”从“湿、瘀”论治带下病学术思想探析

摘　　要：带下病是常见多发妇科病之一,复发率高,继发病种多,严重影响女性患者身心健康。朱宗元教授遵循"瘀与湿不能相分离、治瘀湿自除、久病入络"的理念,认为湿邪是带下病发病的首要条件,其核心病机为脾失健运、肝肾受累,湿浊内蕴,久病湿瘀互结,阻滞经脉,带脉失约,冲任不固,治疗以祛湿化瘀为主,兼以暖脾温肾,临证辅以温阳散寒祛湿等。方药选取薏苡附子败酱散寒温并用入下焦,温阳健脾、清热祛湿化瘀的功效来治疗带下病,充分体现了"谨守病机,异病同治"的思想。朱老在长期临床诊疗形成了独特的学术观点和用药方法,文章阐述朱老治疗带下病的临床经验,并举一病案分享。Morbid vaginal discharge is one of the common gynecological diseases with high recurrence rate and many secondary diseases,which seriously affect the physical and mental health of female patients.Professor ZHU Zong-yuan followed the concept of’blood stasis and dampness can not be separated,the treatment of blood stasis and dampness can be removed by itself,and the long-term disease can enter the collaterals’.He believed that the pathogenic dampness is the primary condition for the onset of the morbid vaginal discharge.The core pathogenesis of the disease is spleen dysfunction,liver and kidney involvement,the accumulation of dampness and turbidness,the combination of dampness and blood stasis for a long time,the blocking of meridians,the loss of contract with the meridians,the unsteadiness of Chong and Ren,the main treatment is to remove dampness and stasis,and the warming of the spleen and the kidney dampness,and so on.The prescription selected Yiyi Fuzi Baijiang Powder with effect of warming yang,invigorating the spleen,clearing away heat,removing dampness and removing blood stasis was used to treat the morbid vaginal discharge,which fully reflected the idea of’observing the pathogenesis,treating different diseases with same method’.Professor ZHU has formed a unique academic point of view and medication method in the long-term clinical diagnosis and treatment.This paper will elaborate professor ZHU clinical experience in the treatment of morbid vaginal discharge,and share a medical record.
