摘 要:目的探究经阴道子宫输卵管三维超声造影(3D-HyCoSy)与实时三维超声造影(RT-3D-HyCoSy)在输卵管通畅性诊断中的应用价值。方法回顾性分析120例不孕症患者临床资料,经3D-HyCoSy与RT-3D-HyCoSy检查,以腹腔镜通液术(LC)结果为诊断金标准,比较两种超声造影诊断结果。结果3D-HyCoSy结果:通畅144条,通而不畅41条,堵塞55条;RT-3D-HyCoSy结果:通畅137条,通而不畅47条,堵塞56条;LC结果:通畅129条,通而不畅54条,堵塞57条。3D-HyCoSy检查结果:通畅诊断符合率81.94%,通而不畅诊断符合率60.98%,堵塞诊断符合率94.55%,一致性Kappa为0.68(P<0.05);RT-3D-HyCoSy检查结果:通畅诊断符合率91.97%,通而不畅诊断符合率85.11%,堵塞诊断符合率94.64%,一致性Kappa为0.85(P<0.05);RT-3D-HyCoSy检查结果通畅、通而不畅符合率均高于3D-HyCoSy检查结果,差异均有统计学意义(χ^2分别=6.17、7.47,P均<0.05)。结论3D-HyCoSy与RT-3DHyCoSy在诊断输卵管通畅性中均有较高诊断符合率,但RT-3D-HyCoSy可储存动态图像且具备较高重复性。Objective To explore the application values of three-dimensional hysterosalpingo-contrast sonography(3D-HyCoSy)and real-time three-dimensional hysterosalpingo-contrast sonography(RT-3D-HyCoSy)in the diagnosis of fallopian tube patency.Methods Clinical data of 120 infertility patients was retrospectively analyzed.All patients were examined by 3D-HyCoSy and RT-3D-HyCoSy,and the results of laparoscopy chromopertubation(LC)as the gold standard,the diagnosis values of the two kinds of contrast-enhanced ultrasound were compared.Results The 3D-HyCoSy results showed that 144 fallopian tubes were patency,41 fallopian tubes were pass and poor,and 55 fallopian tubes were obstruction.Rt-3D-HyCoSy results showed that 137 fallopian tubes were patency,47 fallopian tubes were pass and poor,and 56 fallopian tubes were obstruction.LC results showed that 129 fallopian tubes were patency,54 fallopian tubes were pass and poor,and 57 fallopian tubes were obstruction.The accurency rate of 3D-HyCOSY for diagnosing tubal patency,tubal pass and poor,and tube obstruction were 81.94%,60.98%,and 94.55%,the Kappa was 0.68(P<0.05).The accurency rate of Rt-3D-HyCoSy for diagnosing tubal patency,tubal pass and poor,and tube obstruction were 91.97%,85.11%,and 94.64%,the Kappa was 0.85(P<0.05).The accurency rate of Rt-3D-HyCoSy for diagnosing tubal patency,tubal pass and poor were higher than 3D-HyCoSy(χ^2=6.17,7.47,P<0.05).Conclusion Both 3D-HyCoSy and RT-3DHyCoSy have a high accurency rate in the diagnosis of tubal unobtusability.However,RT-3D-HyCoSy can storing dynamic images and has high repeatability.
