摘　　要：目的回顾性调查分析宝鸡市某三级医院重症监护病房(ICU)导尿管相关医院感染特征,为预防控制医院感染提供参考。方法采用系统回顾性分析法收集2016年1月-2018年12月在宝鸡市某三级医院ICU住院治疗患者的临床病例资料。结果1312例患者中男性830例,女性432例,年龄中位数62.5岁。导尿管相关医院感染率为8.23%,检出病原菌以革兰阴性菌为主。随着术后住院时间增加,导尿管相关医院感染率呈升高趋势。年龄>62.5岁、术后住院天数≥26 d、接受导尿管插管≥10 d、使用呼吸机≥1 d是导致导尿管相关医院感染的独立影响因素。结论ICU住院患者中导尿管相关医院感染率较高,医院采取措施预防控制导尿管相关医院感染时应考虑患者年龄、术后住院时间、接受导尿管插管辅助治疗时间、使用呼吸机时间等影响因素。Objective To retrospectively investigate and analyze the characteristics of catheter-associated nosocomial infections in ICU of a tertiary hospital in Baoji City,so as to provide references for the prevention and control of nosocomial infections.Methods A systematic retrospective analysis method was used to collect clinical case data of patients who were hospitalized in ICU of a tertiary hospital in Baoji City from January 2016 to December 2018.Results Among the 1312 patients,830 patients were male and 432 patients were female,with a median age of 62.5 years.The rate of catheter-associated nosocomial infections was 8.23%,and the main pathogens detected were Gram-negative bacteria.With the increase of postoperative hospital stay,the rate of catheter-associated nosocomial infections showed an increasing trend.Age>62.5 years old,postoperative hospital stay≥26 days,receiving catheter intubation≥10 days,use of ventilator≥1 day were independent influencing factors of catheter-associated nosocomial infections.Conclusion The rate of catheter-associated nosocomial infections in ICU inpatients are relatively high.The hospital takes measures to prevent and control catheter-associated nosocomial infections,including age of patient,postoperative hospital stay,the influencing factors such as time of adjuvant therapy of receiving catheter intubation,time of using ventilator and so on.
