化湿解毒止带方治疗慢性宫颈炎伴HPV感染的临床疗效及对血清hs-CRP、TNF-α水平的影响

摘　　要：目的观察化湿解毒止带方对慢性宫颈炎伴人乳头瘤病毒(HPV)感染的临床疗效及对血清hs-CRP、TNF-α水平的影响。方法选择2017年3月—2018年10月来医院治疗的慢性宫颈炎伴HPV感染患者140例,依据随机数字表法将患者分为研究组和对照组,每组70例。对照组患者采用重组人干扰素α2b栓治疗,研究组患者在对照组治疗的基础上,口服化湿解毒止带汤治疗。两组均连续治疗3个月。比较两组治疗前后中医症候积分和治疗疗效,比较两组治疗前后血清炎症因子水平和免疫球蛋白水平,与治疗后6个月和12个月随访,比较两组复发情况。结果研究组治疗总有效率为91.43%(64/70),对照组治疗总有效率为78.57%(55/70),研究组治疗总有效率高于对照组(P<0.05);研究组治疗后带下量多、色黄质稠、外阴瘙痒以及小腹作痛等临床症状和体征评分低于对照组(P<0.05);研究组治疗后血清炎症因子TNF-α、hs-CRP和IL-6水平低于对照组(P<0.05);研究组治疗后免疫球蛋白IgG、IgM和IgA水平高于对照组(P<0.05);随访治疗后12月研究组的复发率低于对照组(P<0.05)。结论化湿解毒止带汤结合重组人干扰素α2b栓治疗慢性宫颈炎伴HPV感染效果显著,可有效的改善患者的临床症状,降低炎症因子含量,提高免疫力,抑制本病复发,改善预后,值得临床进一步研究探讨。Objective To observe the clinical effect of Huashi Jiedu Zhidai Formula on chronic cervicitis with HPV infection and the effect on serum levels of hs-CRP and TNF-α. Methods A total of 140 patients with chronic cervicitis and HPV infection who were treated in our hospital from March 2017 to October 2018 were selected, and the patients were divided into study group and control group according to the random number table method, with 70 cases in each group. Patients in the control group were treated with recombinant human interferon a2 b suppository, while patients in the study group were treated with Huashi Jiedu Zhidai Formula on the basis of treatment in the control group. Both groups were treated for 3 months. TCM syndrome score and treatment effect before and after treatment were compared between the two groups. Serum inflammatory factors levels and immunoglobulin level before and after treatment were compared between the two groups. Results The total effective rate was 91.43%(64/70) in the study group and that was 78.57%(55/70) in the control group, and the total effective rate in the study group was higher than that in the control group(P<0.05). After treatment, the score of the clinical symptoms and signs of the study group was lower than that of the control group(P<0.05). Serum levels of hs-CRP,TNF-α and IL-6 in the study group were lower than those in the control group(P<0.05). After treatment, levels of IgG, IgM and IgA in the study group were higher than those in the control group(P<0.05). The recurrence rate in the study group was lower than that in the control group(P<0.05). Conclusion Huashi Jiedu Zhidai Formula combined with recombinant human interferon a2 b suppository has a significant effect on treating chronic cervicitis with HPV infection, which can effectively improve the clinical symptoms of patients, reduce the contents of inflammatory factors, improve immunity, inhibit the recurrence of the disease and improve the prognosis. It is worthy of further clinical research and discussion.
