祛风宣肺方治疗感染后咳嗽疗效及诱导痰中炎症细胞比例变化的研究

摘　　要：目的研究祛风宣肺方对感染后咳嗽患者的疗效及诱导痰中炎症细胞比例的变化。方法选取医院2017年3月—2019年3月收治的感染后咳嗽患者122例,根据入院顺序分为两组。对照组给予复方甲氧那明胶囊治疗,观察组在对照组基础上给予祛风宣肺方治疗,分析两组患者治疗后的临床疗效。结果观察组总有效率为90.16%(55/61),对照组为68.85%(42/61),观察组患者总有效率高于对照组(P<0.05)。两组治疗前诱导痰中炎症细胞比例组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后诱导痰中巨噬细胞(MAC)百分比(38.86±11.87)%高于对照组,诱导痰中中性粒细胞(NEU)百分比(54.11±16.52)%、嗜酸性粒细胞(EOS)百分比(0.36±0.15)%低于对照组(P<0.05)。两组治疗前血清炎症因子水平组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后干扰素-γ(INF-γ)(34.85±5.03)ng/L水平高于对照组,白细胞介素-4(IL-4)(32.08±4.52)ng/L、肿瘤坏死因子-α(TNF-α)(8.84±2.89)mg/L水平低于对照组(P<0.05)。两组治疗前咳嗽症状积分组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后日间和夜间咳嗽症状积分低于对照组(P<0.05)。结论祛风宣肺方对可改善感染后咳嗽患者诱导痰中炎症细胞比例,减轻炎症反应,提高治疗效果。Objective To study the curative effect of Qufeng Xuanfei Recipe on cough patients after infection and the changes of inflammatory cells in the induced sputum. Methods A total of 122 patients with post-infection cough treated in our hospital from March 2017 to March 2019 were divided into two groups according to the order of admission. The control group was treated with Compound Methoxymine Capsule, and the observation group was treated with Qufeng Xuanfei Recipe on the basis of the control group. The clinical efficacy of the two groups after treatment was analyzed. Results The total effective rate of the observation group was 90.16%(55/61) and that of the control group was 68.85%(42/61). The total effective rate of the observation group was higher than that of the control group(P<0.05). After treatment, the percentage of macrophage(MAC) in the induced sputum in the observation group[(38.86±11.87)%] was higher than that in the control group, and the percentage of NEU in induced sputum neutrophils was(54.11±16.52)%. The percentage of EOS[(0.36±0.15)%] was lower than that in the control group(P<0.05). There was no significant difference in the levels of serum inflammatory factors between the two groups before treatment(P>0.05). After treatment, the level of interferon-gamma(INF-γ)[(34.85±5.03)ng/L] in the observation group was higher than that in the control group, and the level of IL-4[(32. 08±4. 52) ng/L] in the observation group was higher than that in the control group. The level of tumor necrosis factor-α(TNF-α)[(8.84±2.89)mg/L] was lower than that of the control group(P<0.05). There was no significant difference in cough symptom score between the two groups before treatment(P>0.05). In the observation group during the day after treatment,the score of cough symptoms at night was lower than that of the control group(P<0.05). Conclusion The effects of Qufeng Xuanfei Recipe on the proportion of inflammatory cells in the induced sputum of patients with cough after infection can be improved. The inflammator
