温针灸联合龙胆泻肝汤治疗带状疱疹后遗神经痛的效果

摘　　要：目的分析温针灸联合龙胆泻肝汤对带状疱疹后遗神经痛的效果。方法选取医院2016年1月—2018年9月收治的带状疱疹后遗神经痛患者80例,分为两组。对照组给予西医常规对症治疗,观察组加用温针灸联合龙胆泻肝汤治疗,分析两组患者治疗后的临床疗效。结果观察组总有效率为92.50%(37/40),对照组为75.00%(30/40),观察组患者总有效率高于对照组(P<0.05)。两组治疗前视觉模拟(VAS)评分组间比较,差异不具有统计学意义(P>0.05)。观察组治疗3 d、7 d、14 d后VAS评分(6.58±1.14)分、(4.21±1.18)分、(3.41±0.85)分低于对照组(P<0.05)。两组治疗前白细胞介素-10(IL-10)、β-内啡肽(β-EP)、环氧化酶-2(COX-2)水平组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后β-EP水平高于对照组,IL-10、COX-2水平低于对照组(P<0.05)。两组治疗前匹兹堡睡眠质量指数表(PSQI)评分组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后PSQI评分(5.26±2.05)分低于对照组(P<0.05)。结论温针灸联合龙胆泻肝汤治疗带状疱疹后遗神经痛可减轻患者疼痛,调节IL-10、β-EP、COX-2的表达水平,改善睡眠。Objective To analyze the effect of warming acupuncture combined with Longdan Xiegan Decoction on postherpetic neuropain. Methods Eighty patients with postherpetic neuralgia treated in our hospital from January 2016 to September 2018 were divided into two groups. The control group was treated with routine symptomatic treatment of western medicine and the observation group was treated with warming acupuncture combined with Longdan Xiegan Decoction. The clinical efficacy of the two groups after treatment was analyzed. Results The total effective rate was 92.50% in the observation group and 75.00% in the control group. The total effective rate in the observation group was higher than that in the control group(P<0.05). There was no significant difference in visual analog(VAS) score between the two groups before treatment(P>0.05). The VAS scores of the observation group 3 d,7 d and 14 d after treatment[(6.58±1.14)points,(4.21±1.18)points and(3.41±0.85)points] were lower than those of the control group(P<0.05). Before treatment, levels of IL-10,β-endorphin(β-EP) and COX-2 between the two groups were not significant(P>0.05). After treatment, the level of β-EP in the observation group was higher than that in the control group, and the levels of IL-10 and COX-2 were lower than those in the control group(P<0.05). The PSQI score of Pittsburgh sleep quality index table before treatment between the two groups was not statistically significant(P>0.05). After treatment, the PSQI score of the observation group[(5.26 ±2.05)points] was lower than that of the control group(P<0.05). Conclusion Warming acupuncture combined with Longdan Xiegan Decoction in the treatment of postherpetic neuropain can relieve the pain, regulate the expressions of IL-10, β-EP and COX-2 and improve sleeping.
