摘　　要：阮诗玮教授系福建省名中医,从医三十余年,擅长运用药对治疗慢性肾脏病。阮诗玮教授根据慢性肾脏病临床症候特点及血肌酐、尿蛋白、血小板等实验室相关检测指标结果,采用辨病与辨证相结合、宏观与微观相结合、现代医学与传统医学相结合的辨治理念,在长期的临床实践中积累了丰富的用药经验,形成了"治水必先温通""温阳兼祛邪,善后重温补"等治疗慢性肾脏疾病学术思想。慢性肾脏病病机在于脾肾两脏虚损,久病而正气渐损,卫外不固,失于御外,易感受外邪而出现标证,故而痰浊瘀毒蕴、肺气不宣、上焦郁热、肾失封藏,其症候多为倦怠乏力,腰酸膝软,口干咽燥,五心烦热,夜尿清长,面色萎黄。阮诗玮教授常选用大黄配六月雪、牛蒡子配蝉蜕、升麻配黄连、荷叶配薄荷、沙苑子配覆盆子、白藓皮配地肤子、双钩藤配乌点规、仙茅配淫羊藿、墨旱莲配女贞子、蒲黄配五灵脂、三棱配莪术、茜草配紫草、王不留行配路路通,以求温补脾肾阳、通畅气机、固肾摄精。阮诗玮教授针对慢性肾脏疾病中医学主症及兼症在辨证的基础上做到药症相应,药物成对,从而达到药简功专,事半功倍的治疗效果,文章详述阮教授临证常用13组药对的组成功效、配伍要点、适应病症,对阮教授治疗慢性肾脏病的用药经验做一总结。Professor RUAN Shiwei is a famous traditional Chinese medicine in Fujian Province. He has been practicing medicine for more than 30 years. He is good at treating chronic kidney disease with corresponding medicine. According to the clinical characteristics of chronic kidney disease and the results of laboratory related detection indexes such as serum creatinine,urinary protein and platelet,Professor RUAN Shiwei has accumulated rich medication experience in long-term clinical practice by combining disease differentiation with syndrome differentiation,macro and micro combination,modern medicine and traditional medicine " Dispel pathogenic factors,take good care of the aftercare,re warm and tonic" and other academic ideas on the treatment of chronic kidney disease. The pathogenesis of chronic kidney disease lies in the deficiency of spleen and kidney,chronic disease and progressive loss of healthy Qi,the external Wei is not solid,lost in the external defense,easy to feel the external evil and appear the standard syndrome,therefore,phlegm and blood stasis and toxin accumulation,Lung Qi is not clear,upper Jiao is hot,kidney is not sealed,its symptoms are fatigue and fatigue,waist is sour and knee is soft,mouth is dry and throat is dry,five heart is vexed and hot,night urine is long,complexion is pale. Professor RUAN Shiwei often chooses rhubarb with junyuexue,Arctium lappa with cicada slough,Cimicifuga with Coptis,lotus leaf with mint,shayuanzi with raspberry,white moss skin with Kochia,shuanggouteng with Qilikou,Xianmao with Xianlingpi,Eclipta with Nuzhen Zi,Puhuang with wulingzhi,Sanleng with zedoary,Rubia with Zicao,wangbuliuxing with Lutong,in order to warm spleen and kidney yang,unblock Qi and strengthen kidney Ejaculation. Professor RUAN Shiwei aimed at the main and concurrent symptoms of chronic kidney disease in traditional Chinese medicine,on the basis of syndrome differentiation,to achieve the corresponding medicine and symptoms,drugs in pairs,so as to achieve the therapeutic effect of simple and specialized
