摘 要:目的探讨不同感染性疾病患者外周血中T、CD3^+CD4^+T、CD3^+CD8^+T、CD4^-CD8^-T、CD4^+CD8^+T、B、NK、CD4^+Treg、CD8^+Treg、CTL及CD28^-CTL等淋巴细胞亚群的表达水平,为不同感染性疾病的辅助诊断、免疫监测以及临床治疗提供依据。方法选取本院2017年10月-2018年2月诊治的不同类型感染性疾病86例,其中败血症26例,HBV感染30例,EBV感染30例,对照组30例;用流式细胞术检测各组淋巴细胞亚群表达水平。结果与对照组相比,HBV组、EBV组、败血症组患者的外周血CD3^+CD4^+T、CD3^+CD8^+T、CD4^+CD8^+T、B、NK、CD4^+Treg、CTL及CD28^-CTL等淋巴细胞亚群的表达水平差异具有统计学意义(P<0.05),HBV组中CD3^+CD4^+T和CTL升高,NK和CD28^-CTL降低;EBV组中CD3^+CD8^+T、CD4^+Treg和CTL升高,CD3^+CD4^+T和NK降低;败血症组中B和CTL升高,CD4^-CD8^-T、NK和CD28^-CTL降低。与败血症和HBV组相比,EBV组的CD3^+CD4^+T表达降低,CD3^+CD8^+T和CTL表达升高,差异具有统计学意义(P<0.05);与HBV组相比,败血症组CD4^-CD8^-T表达降低,与EBV组相比,败血症组B表达升高,HBV组CD28^-CTL降低,差异具有统计学意义(P<0.05)。另外,比较发现HBV组、EBV组、败血症组三组患者的外周血中均表现为CTL表达升高,NK表达降低。结论不同感染性疾病患者外周血中CD3^+CD4^+T、CD3^+CD8^+T、CD4^-CD8^-T、B、NK、CD4^+Treg、CTL及CD28^-CTL等淋巴细胞亚群的表达水平存在差异,通过分析其表达水平的变化可为不同感染性疾病的辅助诊断、免疫监测以及临床治疗提供依据。Objective To investigate the expression level of cells including T,CD3^+CD4^+T,CD3^+CD8^+T,CD4^-CD8^-T,CD4^+CD8^+T,B,NK,CD4^+Treg,CD8^+Treg,CT,CD28^-CTL in peripheral blood of patients with different infectious diseases,so as to provide experimental evidence for the auxiliary diagnosis,immune monitoring,and clinical treatment of different infectious diseases.Methods Eightysix cases of different types of infectious diseases diagnosed and treated in our hospital from 2017 to 2018 were selected,including 26 cases of septicemia,30 cases of HBV and 30 cases of EBV,and in addition to 30 cases of control group.Flow cytometry was used to detect the expression of lymphocyte subsets in each group.Results Compared with the control group,the expression levels of lymphocyte subsets CD3^+CD4^+T,CD3^+CD8^+T,CD4^+CD8^+T,B,NK,CD4^+Treg,CTL,CD28^-CTL in patients with HBV,EBV and septicemia were significantly different(P<0.05).In HBV group,CD3^+CD4^+T and CTL were elevated while NK and CD28^-CTL decreased;In HBV group,CD3^+CD8^+T,CD4^+Treg and CTL increased while CD3^+CD4^+T and NK decreased;In septicemia group,B and CTL increased while CD4^-CD8^-T,NK and CD28^-CTL decreased.Compared with the control group,the expression levels of peripheral blood T,CD4^+CD8^+T,CD8^+Treg in patients with HBV,EBV and septicemia had no significant difference(P>0.05).Compared with septicemia and HBV group,the expression of CD3^+CD4^+T in EBV group decreased while the expression of CD3^+CD8^+T and CTL increased.The difference was statistically significant(P<0.05);Compared with The HBV group,the expression of CD4^-CD8^-T in the septicemia group decreased.Compared with the EBV group,the expression of B in the septicemia group increased.The number of CD28^-CTL in the HBV group decreased,and the difference was statistically significant(P<0.05).In addition,it was found that the expression of CTL in the peripheral blood of the HBV group,EBV group,and septicemia group showed increased while decreased in NK.Conclusion Compared with healthy people,the expressio
