妊娠合并子宫肌瘤行剖宫产术中肌瘤切除的临床疗效研究

摘　　要：目的探讨妊娠合并子宫肌瘤行剖宫产术中肌瘤切除临床效果。方法以2018年1月—2019年10月作为研究时间范围,方便选取90例该院收治妊娠合并子宫肌瘤患者作为分析对象,采用随机法将90例患者分为对照组与实验组,其中采用常规行子宫下段剖宫产术45例患者作为对照组,采用剖宫产术中肌瘤切除术45例患者作为实验组,对比实验组与对照组患者治疗效果。结果实验组患者术中出血量(375.21±11.82)mL、术后出血量(89.62±9.25)m L、术后宫缩使用量(30.23±3.57)U与对照组相比差异无统计学意义(t=1.687、1.682、1.464,P=0.095、0.066、0.147),实验组患者手术时间(87.12±12.25)min以及疼痛度大于对照组患者,差异有统计学意义(t=8.651、6.844、5.883、2.704,P<0.05)。实验组患者术后并发症发生率17.77%与对照组13.33%相比,差异无统计学意义(χ~2=0.338,P=0.561)。结论妊娠合并子宫肌瘤患者应用剖宫产术中肌瘤切除术效果显著,可以减少患者二次手术,具有较高安全性。Objective To explore the clinical effect of fibroid resection during cesarean section during pregnancy with uterine fibroids.Methods Taking January 2018 to October 2019 as the study time range,90 patients with pregnancy and uterine fibroids admitted in the hospital were conveniently selected as the analysis object,and 90 patients were randomly divided into the control group and the experimental group,of which the routine 45 patients who underwent cesarean section under the uterus were used as the control group,and 45 patients who underwent myomectomy during the cesarean section were used as the experimental group.Results There was no statistically significant difference in the experimental group intraoperative blood loss(375.21±11.82)mL,postoperative blood loss(89.62±9.25)mL,and postoperative uterine contraction(30.23±3.57)U compared with the control group(t=1.687,1.682,1.464,P=0.095,0.066,0.147).The operation time(87.12±12.25)min and pain degree of the experimental group were greater than those of the control group,the difference was statistically significant(t=8.651,6.844,5.883,2.704,P<0.05).The postoperative complication rate of the experimental group was 17.77%and the control group 13.33%had no statistically significant difference(χ2=0.338,P=0.561).Conclusion The use of fibroidectomy during cesarean section in pregnant patients with uterine fibroids has a significant effect.It can reduce the patients'secondary operations,has high safety.
