经尿道膀胱颈电切术用于梗阻性慢性前列腺炎治疗的临床效果分析

摘　　要：目的探究分析在梗阻性慢性前列腺炎治疗中应用经尿道膀胱颈电切术治疗的临床效果。方法便利选取该院于2018—2019年期间收治的56例梗阻性慢性前列腺炎患者为该次研究对象,对其采取经尿道膀胱颈电切术治疗,观察对比手术治疗前后患者的各项临床指标和生活质量评分以及术后不良反应发生情况,以此来判定该手术治疗梗阻性慢性前列腺炎的临床疗效。结果相较于手术治疗前,治疗后,患者的最大尿流率(Qmax)、残余尿流量(RUV)以及前列腺症状评分(IPSS)分别由(14.98±0.97)mL/min、(24.79±5.12)mL、(19.68±4.88)分变为(20.85±2.03)mL/min、(9.57±1.73)mL、(6.15±2.12)分,差异有统计学意义(t=19.525、21.075、19.303, P<0.05),各项临床指标皆得到有效改善,(GQOLI-74)生活质量评分更高,差异有统计学意义(t=26.051,P<0.05)。虽然术后发生了不良反应,但仅发生6例,占比10.7%,通过对症治疗得到良好控制。结论在梗阻性慢性前列腺炎治疗中应用经尿道膀胱颈电切术治疗,能够有效改善各项临床指标,解除梗阻,改善治疗预后效果,提高生活质量,减少不良反应的发生,有助于促进患者恢复,具有较高临床应用价值。Objective To explore and analyze the clinical effect of transurethral resection of bladder neck in the treatment of obstructive chronic prostatitis.Methods 56 patients with obstructive chronic prostatitis admitted in the hospital from 2018 to 2019 were selected as the data source of this study.The patients were treated by transurethral resection of bladder neck.The clinical indexes,quality of life scores and adverse reactions were observed and compared before and after the operation,so as to determine the effect of the operation on obstructive chronic prostatitis clinical efficacy.Results Compared with before and after the operation,the patients'Qmax,RUV and IPSS changed from(14.98±0.97)mL/min,(24.79±5.12)mL,(19.68±4.88)points to(20.85±2.03)mL/min,(9.57±1.73)mL,(6.15±2.12)points respectively.The difference was statistically significant(t=19.525,21.075,19.303,P<0.05).All the clinical indexes were improved effectively.GQOLI-74 had a higher quality of life score,and the difference was statistically significant(t=26.051,P<0.05).Although there were adverse reactions after operation,only 6 cases(10.7%)occurred,which were well controlled by symptomatic treatment.Conclusion In the treatment of obstructive chronic prostatitis,transurethral resection of the bladder neck can effectively improve the clinical indicators,relieve obstruction,improve the prognosis,improve the quality of life,reduce the incidence of adverse reactions,and help to promote the recovery of patients.It is a kind of surgical treatment with high clinical application value.
