自拟解毒凉血燥湿方辅助治疗乙型肝炎慢性肝衰竭45例疗效观察

摘　　要：目的:观察自拟解毒凉血燥湿方辅助治疗乙型肝炎慢性肝衰竭的疗效及对凝血功能的影响。方法:选取2018年1月-2020年1月本院收治的乙型肝炎慢性肝衰竭患者90例,采用随机数字表法将其分为对照组和观察组各45例。对照组行西医综合治疗,观察组在对照组基础上加用自拟解毒凉血燥湿方辅助治疗,对比两组临床效果和肝功能、凝血功能治疗前后变化情况以及血清病毒学指标、AFP变化。结果:观察组和对照组的临床有效率分别为91.11%和73.33%,观察组明显高于对照组(P<0.05)。与治疗前相比,两组谷草转氨酶(AST)、血清谷氨酸氨基转移酶(ALT)和国际标准化比值(INR)均下降明显,凝血酶原活动度(PTA)显著升高(P<0.05),且观察组的升降幅度显著大于对照组(P<0.05)。治疗后两组乙型肝炎e抗原(HBeAg)、甲胎蛋白(AFP)以及乙肝病毒脱氧核糖核酸(HBVDNA)含量均较治疗前显著下降(P<0.05),且观察组上述指标均明显低于对照组(P<0.05)。结论:自拟解毒凉血燥湿方辅助治疗乙型肝炎慢性肝衰竭,可改善患者的肝功能及凝血功能,控制病毒复制,效果显著。Objective:To observe the effect of Jiedu Liangxue Zaoshi Decoction on chronic liver failure of hepatitis B and its influence on coagulation function.Methods:90 patients with chronic liver failure of hepatitis B in our hospital from January 2018 to January 2020 were selected and randomly divided into control group and observation group with 45 cases in each group.The control group was treated with western medicine,while the observation group was treated with Jiedu Liangxue Zaoshi Decoction on the basis of the control group.The clinical effect,liver function and coagulation function before and after treatment,as well as the changes of serum virological indexes and AFP were compared between the two groups.Results:The clinical effective rates of the observation group and the control group were 91.11%and 73.33%,respectively.The observation group was significantly higher than the control group(P<0.05).Compared with before treatment,AST,ALT and INR of the two groups were significantly decreased,and prothrombin activity(PTA)was significantly increased(P<0.05),and the improvement of the observation group was significantly greater than that of the control group(P<0.05).After treatment,the contents of HBeAg,AFP and HBV-DNA in the two groups were significantly decreased(P<0.05),and the above indexes in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:It can improve the liver function and coagulation function of patients with chronic liver failure of hepatitis B,and control virus replication.
