摘　　要：目的分析尿肝素结合蛋白(U-HBP)和尿白细胞酯酶(LEU)检测在经尿道前列腺切除术(TURP)后尿路感染(UTI)诊断中的应用价值。方法随机选取2017年2月至2019年6月武汉科技大学附属天佑医院诊治的205例接受TURP治疗的良性前列腺增生患者作为研究对象。按照中段尿液细菌培养结果将患者分为UTI组(n=28)、可疑UTI组(n=17)、对照组(n=160)。检测三组患者的U-HBP、LEU水平,并进行组间比较;根据受试者操作特征(ROC)曲线计算U-HBP、LEU及U-HBP+LEU对术后UTI诊断的灵敏度、特异度、阳性预测值、阴性预测值及准确度。结果UTI组患者的U-HBP水平明显高于其他两组,差异具有统计学意义(P<0.05)。UTI组患者的LEU的检测结果与其他两组相反。U-HBP+LEU诊断术后UTI的灵敏度和特异度最高,分别为91.09%和91.98%。结论U-HBP+LEU检测TURP后UTI的灵敏度、特异度较好,而且方便快捷,可以作为诊断TURP后UTI的指标。Objective To investigate the value of uro-heparin-binding protein(U-HBP)and urinary leucocyte esterase(LEU)in diagnosing urinary tract infection(UTI)after transurethral resection of prostate(TURP).Methods A total of 205 patients with benign prostatic hyperplasia,treated with TURP in Tianyou Hospital Affiliated to Wuhan University of Science and Technology from February 2017 to June 2019,were randomly selected.According to the results of urine bacteria culture in the middle section,patients were divided into UTI group(n=28),suspected UTI group(n=17)and control group(n=160).The levels of U-HBP and LEU in three groups were detected and compared between groups.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of U-HBP,LEU and U-HBP+LEU for postoperative UTI diagnosis were calculated according to the ROC curve.Results U-HBP level in UTI group was significantly higher than that in the other two groups,and the difference was statistically significant(P<0.05).The LEU test results of UTI group were statistically significant compared with the other two groups.The sensitivity and specificity of UTI after U-HBP+LEU diagnosis were 91.09%and 91.98%,respectively.Conclusions U-HBP+LEU has good sensitivity,specificity and convenience in the detection of UTI after TURP,which can be used as an indicator to diagnose UTI after TURP.
