妊娠期和产后严重贫血孕产妇不良妊娠结局风险的多水平分析研究

摘　　要：目的探讨妊娠期和产后严重贫血孕产妇不良妊娠结局的风险。方法选取2010年1月至2017年12月深圳市坪山区人民医院接收的200例贫血孕产妇作为研究对象。记录患者年龄、体重指数(BMI)、受教育程度、收入水平、孕次、职业、孕期饮酒、孕期吸烟等特征,纳入病例依据其诊断为贫血时是处于妊娠期还是生产后而分为妊娠组(n=100)和产后组(n=100)。分别采用单因素分析及Logistic多因素分析对妊娠期和产后严重贫血孕产妇不良妊娠结局的风险因素进行统计分析。结果单因素分析结果显示,妊娠组孕产妇不良妊娠结局与受教育程度、职业、收入水平、孕期吸烟和补充铁剂相关,差异具有统计学意义(P<0.05),产后组孕产妇不良妊娠结局与产前BMI、生产次数、职业及孕期饮茶相关,差异具有统计学意义(P<0.05)。Logistic多因素分析结果显示,妊娠组严重贫血孕产妇不良妊娠结局的危险因素包括受教育程度、收入水平、职业、孕期吸烟以和补充铁剂,产后组严重贫血孕产妇不良妊娠结局的危险因素包括职业、生产次数以及孕期饮茶。结论严重贫血孕产妇的不良妊娠结局危险因素包括:职业等个人情况、孕期吸烟和饮茶等生活习惯以及补充铁剂。Objective To explore the risk of adverse pregnancy outcomes in women with severe anemia during pregnancy and postpartum.Methods From January 2010 to December 2017,200 pregnant women in Shenzhen Pingshan People's Hospital were selected as the research objects.The age,body mass index(BMI),education level,income level,pregnancy times,occupation,drinking during pregnancy and smoking during pregnancy were recorded.According to the diagnosis of anemia,the patients were divided into pregnancy group(100 cases)and postpartum group(100 cases).The risk factors of adverse pregnancy outcomes in pregnant women with severe anemia during pregnancy and postpartum period were statistically analyzed by single factor analysis and logistic multivariate analysis.Results The results of univariate analysis showed that there were significant differences in education level,occupation,income level,smoking during pregnancy and iron supplement of adverse pregnancy outcomes in the pregnancy group(P<0.05).The prenatal BMI value and production times of adverse pregnancy outcomes in the production group were statistically significant(P<0.05).The results of logistic multivariate analysis showed that the risk factors of adverse pregnancy outcomes of pregnant women with severe anemia in pregnanc group included education level,income level,occupation,smoking during pregnancy and iron supplement.The risk factors of adverse pregnancy outcomes of pregnant women with severe anemia in postpartum group included occupation,frequency of occurrence and drinking tea during pregnancy.Conclusions Risk factors for adverse pregnancy outcome in women with severe anemia include personal characteristics such as occupation,habits such as smoking and drinking tea during pregnancy,and iron supplementation.
