摘  要:目的分析内耳磁共振(MR)水成像对良性阵发性位置性眩晕(BPPV)的临床诊断价值。方法选取我院收治的160例BPPV患者,均接受内耳MR水成像与颞骨高分辨率CT检查,比较这两种检查方式对BPPV的阳性检出率。结果高分辨率CT共检出99例(61.88%)BPPV,表现类型为耳石脱落(29.38%)、半规管受累(11.25%)、听小骨受损或者消失(21.25%);内耳MR水成像共检出141例(88.13%)BPPV,表现类型为耳石脱落(41.25%)、半规管变性(20.00%)、半规管破裂(10.63%)及壶腹嵴变薄(16.25%);内耳MR水成像对BPPV的阳性检出率为88.13%,显著高于高分辨率CT(61.88%,P<0.05)。结论内耳MR水成像对BPPV具有较高检出率,能够为患者病情评估与治疗方案选择提供有效指导。Objective To analyze the clinical diagnostic value of inner ear magnetic resonance(MR)water imaging for benign paroxysmal positional vertigo(bPPV).Methods A total of 160 patients with bPPV who were admitted to the hospital were enrolled.All underwent inner ear MR water imaging and temporal bone high-resolution computed tomography(CT).The positive detection rates of the two methods for bPPV were compared.Results A total of 99 cases(61.88%)with bPPV were detected by high-resolution CT.The performance types included otolith shedding(29.38%),semicircular canal involved(11.25%),ossicular damaged or disappearing(21.25%).141 cases(88.13%)with bPPV were detected by inner ear MR water imaging.The performance types included otolith shedding(41.25%),degeneration of semicircular canal(20.00%),rupture of the semicircular canal(10.63%),and crista ampullaris attenuating(16.25%).The positive detection rate of inner ear MR water imaging for bPPV was significantly higher than that of high-resolution CT(88.13%vs 61.88%)(P<0.05).ConclusionThe detection rate of inner ear MR water imaging is relatively higher for bPPV,providing effective guidance for disease assessment and choice of treatment options.
