超声诊断妊高症与新生儿缺血缺氧性脑病的临床价值分析

摘　　要：目的探讨超声诊断新生儿缺血缺氧性脑病临床价值。方法选取我院正常分娩的62例妊高症产妇作为研究对象,同时另选取本院同期正常的60例产妇作为正常对照组,通过超声诊断新生儿缺血缺氧性脑病的发生情况并监测大脑中动脉的血流动力学指标。结果妊高症孕妇组新生儿缺血缺氧性脑病的发生率明显高于对照组的发生率,差异具有统计学差异(χ^2=2.381,P<0.05),并且随着妊高症的严重程度,HIE的发生率以及严重程度也相应提升。妊高症组大脑中动脉的血流动力学指标Vs及Vd值均低于正常对照组,PI、RI值均高于正常对照组,差异具有统计学差异(P<0.05)。结论早期诊断新生儿缺血缺氧性脑病并提供大脑中动脉的血流动力学指标参数,提示新生儿缺血缺氧性脑病的发生率与妊高症有一定的关联。Objective To investigate the clinical correlation between pregnancy induced hypertension(PIH) and neonatal hypoxic-ischemic encephalopathy(HIE) by ultrasound diagnosis. Methods 62 cases of pregnant women with pregnancy induced hypertension(PIH) in our hospital were selected as the research objects, and 60 cases of normal pregnant women in our hospital were selected as the normal control group.Neonatal brain ultrasound examination was completed on the third day of birth. The incidence of hypoxic-ischemic encephalopathy was diagnosed by ultrasound and the hemodynamic indexes of middle cerebral artery were monitored. Results The incidence of neonatal hypoxic-ischemic encephalopathy in PIH group was significantly higher than that in control group(χ^2=2.381, P<0.05), and with the severity of PIH, the incidence and severity of HIE also increased. The hemodynamic indexes vs and VD of middle cerebral artery in PIH group were lower than those in normal control group, PI and RI values were higher than those in normal control group(P<0.05). Conclusion Early diagnosis of neonatal hypoxic-ischemic encephalopathy by ultrasound and provide hemodynamic parameters of middle cerebral artery suggest that the incidence of neonatal hypoxic-ischemic encephalopathy is related to pregnancy induced hypertension, so as to strengthen the treatment and management of pregnant women with pregnancy induced hypertension, to reduce the occurrence of neonatal hypoxic-ischemic encephalopathy, and to effectively improve the prognosis of newborns.
