超声引导下经皮静脉球囊扩张术治疗动静脉内瘘狭窄的临床效果研究

摘　　要：目的分析在动静脉内瘘(AVF)狭窄疾病治疗中超声引导下经皮静脉球囊扩张术(PTA)的临床疗效。方法选取2019年3月至2020年3月该院收治的进行AVF狭窄治疗患者30例,采用随机分组法分为观察组和对照组,每组15例。对照组给予外科手术治疗方法,观察组给予PTA治疗方法。观察两组患者术后内瘘畅通率、术后并发症发生率、狭窄处内径、透析血流速度及内瘘自然流量。结果观察组患者术后3、6个月内瘘畅通率均低于对照组,但差异均无统计学意义(P>0.05);并发症发生率明显低于对照组,差异有统计学意义(P<0.05);两组患者透析血流速度、内瘘自然流量比较,差异均无统计学意义(P>0.05)。结论在AVF狭窄疾病治疗中应用超声引导下PTA治疗方法有助于提升患者术后内瘘畅通率,降低并发症发生率,有效解除了AVF狭窄,临床疗效显著。Objective To analyze the clinical effects of ultrasound-guided percutaneous venous balloon dilation(PTA)in the treatment of arteriovenous fistula(AVF)stenosis.Methods A total of 30 patients who underwent arteriovenous fistula stenosis treatment in the hospital from March 2019 to March 2020 were selected.The patients were divided into 2 groups by randomization,15 cases in each group.The control group was given surgical treatment,and the observation group was given PTA treatment.Observe the patency rate of the internal fistula,the incidence of postoperative complications,the internal diameter of the stenosis,the dialysis blood flow velocity and the natural flow of the internal fistula in the two groups.Results The fistula patency rate of the observation group was lower than that of the control group within 3 months and 6 months after operation,and the difference was not statistically significant(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,and the inner diameter of the stenosis and the blood flow velocity of the dialysis in the observation group were higher than those in the control group.The difference was statistically significant(P<0.05).Conclusion The application of ultrasound-guided percutaneous venous balloon dilation in the treatment of arteriovenous fistula stenosis,It is helpful to improve the unobstructed rate of the patient′s internal fistula,reduce the incidence of complications,and effectively relieve the arteriovenous fistula stenosis.The clinical treatment effect of the disease is significant.
