杂交手术治疗累及弓部的急性主动脉综合征的临床研究

摘　　要：目的研究杂交手术治疗累及弓部的急性主动脉综合征的临床疗效。方法回顾性分析2016年1月~2019年6月我院收治的23例累及弓部病变的急性主动脉综合征患者的临床资料,所有患者均实施腔内修复术并杂交手术治疗,对患者进行跟踪随访并观察治疗效果。结果 23例患者均成功完成腔内修复术并杂交手术,置入覆膜支架;术后未出现脑卒中、截瘫等并发症,术后随访提示人工血管通畅,吻合口无狭窄。截止到目前无死亡病例,恢复良好,无不良事件发生。结论对累及弓部的急性主动脉综合征患者实施杂交手术,扩大腔内修复术手术适应证,临床疗效好,手术成功率高,具有较好的近、中期疗效。Objective To study the clinical effect of hybrid operation on acute aortic syndrome involving the arch. Methods The clinical data of 23 cases with acute aortic syndrome involving the arch from January 2016 to June 2019 were analyzed retrospectively. All cases were treated with endovascular aortic repair and hybrid surgery. The patients were followed up and the therapeutic effect was observed. Results Twenty-three cases were successfully performed endovascular repair and hybrid surgery, and covered stent graft;No complications such as stroke and paraplegia occurred after operation;The follow-up of 6~30 months after operation showed that the artificial blood vessel was unobstructed and anastomosis was unobstructed. There have been no deaths so far,the patients recovered,and no adverse events occurred. Conclusion Hybrid operation in patients with acute aortic syndrome involving the arch can extend indications of endovascularl repair, has a high success rate,and attain good short-term and medium-term curative effect.
