多元叠加复合声治疗慢性主观性耳鸣的临床疗效分析

摘　　要：目的探讨多元叠加复合声治疗慢性主观性耳鸣的临床疗效,以及不同影响因素对治疗效果的影响。方法研究2018年8月~11月期间在我院耳鼻喉门诊就诊且符合入选标准经其他保守治疗无效的70例慢性主观性耳鸣患者,对其进行10 d的多元叠加复合声治疗,观察疗效,以耳鸣残疾量表(tinnitus handicap inventory,THI)评分作为疗效评价标准。结果 (1)经过多元叠加复合声治疗后,70例患者THI得分较治疗前明显降低,差异具有统计学意义(P<0.05);(2)随访10 d、30 d THI得分较治疗前明显降低,且治疗后和随访10 d、30 d THI两两比较无统计学意义(P>0.05);(3)年龄、性别、耳鸣侧别、吸烟、饮酒及是否伴有听力下降在治疗前、后的THI评分的差异均无统计学意义(P>0.05);(4)耳鸣位点的耳鸣主调声频率、纯音测听曲线类型对疗效均无统计学意义(P>0.05);(5)54例伴有听力下降患者中听力下降类型对疗效比较具有统计学意义(P<0.05)。结论多元叠加复合声治疗对慢性主观性耳鸣患者的短期疗效稳定,能降低THI评分,对患者起到改善耳鸣、减轻烦恼、提高生活质量的作用。Objective To explore the clinical effect of tinnitus multivariate integrated sound therapy(T-MIST) on chronic subjective tinnitus and its influence factors. Methods Seventy patients with chronic subjective tinnitus who met the inclusion criteria and were ineffective by other conservative treatments in the otorhinolaryngology clinic of our hospital from August 2018 to November 2018 were enrolled. The patients were treated with T-MIST for 10 days and the curative effect was observed. The tinnitus handicap inventory(THI) score was used as the evaluation standard. Results(1)After T-MIST, the THI score of 70 patients decreased significantly before treatment,and the difference was statistically significant(P<0.05);(2)The THI scores of 10 days of follow-up and 30 days of follow-up were significantly lower than those before treatment, and there was no statistically significant difference in THI after treatment and 10 days follow-up and 30 days of follow-up(P>0.05);(3)There were no significant differences in age, gender, tinnitus, smoking, drinking, and hearing loss in THI before and after treatment(P>0.05);(4)There was no significant difference in the frequency of tinnitus dominant tone at tinnitus site and hearing curve type on the curative effect(P>0.05);(5)The type of hearing loss in 54 patients with hearing loss was statistically significant(P<0.05). Conclusion The short-term curative effect of T-MIST on patients with chronic subjective tinnitus is stable, the THI score can be reduced, tinnitus and annoyance can be relieved, and the quality of life of patients can be improved.
