摘　　要：目的:探究纤维支气管镜肺泡灌洗联合机械通气对重症肺部感染患者血清降钙素原(PCT)水平的影响。方法:2016年3月-2019年11月收治重症肺部感染患者94例,随机分为两组,各47例。对照组采用机械通气治疗;观察组在机械通气基础上联合纤维支气管镜肺泡灌洗治疗。比较两组血清PCT和血管紧张素Ⅱ(AngⅡ)水平、临床症状改善情况和治疗效果。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组发热缓解时间和咳嗽、咳痰消失时间均短于对照组,差异有统计学意义(P<0.05)。观察组治疗后血清PCT和AngⅡ水平均低于对照组,差异有统计学意义(P<0.05)。结论:重症肺部感染患者经机械通气与纤维支气管镜肺泡灌洗联合治疗,可有效改善临床症状,调节血清PCT水平,效果良好。Objective:To investigate the effect of fibro bronchoalveolar lavage combined with mechanical ventilation on serum procalcitonin(PCT)levels in patients with severe pulmonary infection.Methods:From March 2016 to November 2019,94 cases of patients with severe pulmonary infection were enrolled,they were randomly divided into two groups with 47 cases in each group.The control group was treated with mechanical ventilation,while the observation group was treated with fibro bronchoalveolar lavage on the basis of mechanical ventilation.The serum PCT and AngⅡlevels,clinical symptom improvement and therapeutic effect were compared between the two groups.Results:The total effective rate of treatment in observation group was higher than that in control group,the difference was statistically significant(P<0.05).The fever relief time and cough and expectoration disappearance time in the control group were higher than those in the observation group,the differences were statistically significant(P<0.05).After treatment,the serum PCT and AngⅡlevels in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Mechanical ventilation combined with fibro bronchoalveolar lavage can effectively improve the clinical symptoms and regulate the serum PCT level in patients with severe pulmonary infection,the effect is good.
