胸腔闭式引流术与胸腔引流管结合微管治疗气胸的疗效比较

摘　　要：目的:探讨胸腔闭式引流术与胸腔引流管结合微管治疗气胸的疗效。方法:2016年4月-2020年5月收治气胸患者68例,根据不同引流方式分为两组,各34例。对照组给予胸腔闭式引流术治疗;试验组给予胸腔引流管结合微管治疗。比较两组临床疗效。结果:试验组治疗后血清CRP、TNF-α水平及VAS评分均显著低于对照组,差异有统计学意义(P<0.05)。试验组治疗后PaO2、FEV1、VR水平均显著高于对照组,PaCO2水平显著低于对照组,差异有统计学意义(P<0.05)。试验组带管时间、肺复张时间、气胸闭合时间、切口愈合时间及住院时间均显著低于对照组,差异有统计学意义(P<0.05)。结论:与常规胸腔闭式引流术相比,胸腔引流管结合微管治疗气胸疗效更佳,可有效减轻炎症反应,降低疼痛程度,改善肺功能,促进患者病情快速恢复。Objective:To investigate the different effects of closed thoracic drainage and thoracic drainage tube combined with microtubule in the treatment of pneumothorax.Methods:From April 2016 to May 2020,68 cases of patients with pneumothorax were enrolled,they were divided into two groups according to different drainage methods,with 34 cases in each group.The control group was treated with closed thoracic drainage,while the experimental group was treated with thoracic drainage tube combined with microtubule.The therapeutic effects between the two groups was compared.Results:After treatment,the serum CRP,TNF-αlevel and the VAS scale score of the experimental group were significantly lower than those of the control group,the differences were statistically significant(P<0.05).The PaO2,FEV1,VR level of the experimental group were significantly higher than that of the control group and the PaCO2 level was significantly lower than that of the control group,the differences were statistically significant(P<0.05).The time with tube,the time of lung retense,closed time of pneumothorax,incision healing time and the length of hospital stays were significantly lower in the experimental group than in the control group,the differences were statistically significant(P<0.05).Conclusion:Compared with conventional closed thoracic drainage,thoracic drainage tube combined with microtubule is more effective in treating pneumothorax,it can not only effectively reduce the inflammatory response,reduce the degree of pain and improve lung function,but also can promote the rapid recovery of patients'condition.
