摘　　要：目的:探讨无创呼吸机辅助通气治疗慢性阻塞性肺疾病急性加重期(AECOPD)合并Ⅱ型呼吸衰竭患者的临床疗效及对血清HSP27和HSP70水平的影响。方法:2017年10月-2018年5月收治AECOPD合并Ⅱ型呼吸衰竭患者40例,在抗感染解痉平喘治疗基础上联合无创呼吸机辅助治疗,观察治疗效果,同时观察血清中HSP27、HSP70水平变化情况。结果:40例患者治疗后PaO2水平明显升高,PaCO2水平明显降低,与治疗前比较,差异有统计学意义(P<0.05)。治疗后血清HSP27和HSP70水平较治疗前明显下降,差异有统计学意义(P<0.05)。结论:无创呼吸机辅助通气治疗AECOPD合并Ⅱ型呼吸衰竭患者的临床疗效显著,且可以降低血清HSP27和HSP70水平。Objective:To explore the clinical effect of non-invasive ventilator on acute exacerbation of chronic obstructive pulmonary disease complicated with typeⅡrespiratory failure and its influence on serum HSP27 and HSP70.Methods:From October 2017 to May 2018,40 cases of patients with AECOPD and typeⅡrespiratory failure were enrolled,all patients were given non-invasive ventilator-assisted therapy on the basis of anti-infection,antispasmodic and antiasthmatic,the therapeutic effect was observed and the changes of HSP27,HSP70 levels in serum were observed at the same time.Results:After treatment,PaO2 levels were significantly increased in 40 patients and PaCO2 levels were significantly decreased,the differences were statistically significant(P<0.05).The serum HSP27 and HSP70 levels were also decreased significantly after treatment,the differences were statistically significant(P<0.05).Conclusion:Non-invasive ventilator is effective in the treatment of AECOPD with typeⅡrespiratory failure,it can reduce serum HSP27 and HSP70 levels of patients.
