早期血浆二胺氧化酶D-乳酸与急性胃肠损伤的相关性分析

摘　　要：目的探讨早期血浆二胺氧化酶(DAO)、D-乳酸水平与急性胃肠损伤(AGI)严重程度的相关性,并获取其最佳截断值。方法选取2016年4月至2019年4月AGI患者151例,根据AGI诊断分级标准,分为Ⅰ级组、Ⅱ级组、Ⅲ级和Ⅳ级组,入组24h内采取静脉血,检测血浆DAO与D-乳酸的水平。结果四组患者的DAO和D-乳酸水平分别为(3.35±2.03)U/L、(25.83±13.76)U/L(1级组),(14.92±17.33)U/L、(47.71±19.96)U/L(Ⅱ级组),(30.22±11.25)U/L、(59.23±14.08)U/L(Ⅱ级组),(53.84±24.54)U/L、(68.09±12.85)U/L(IV级组),四组间差异有统计学意义(P<0.05)。相关系数r为0.741(95%CI 0.672~0.825,P<0.01)和0.681(95%CI 0.589-0.758,P<0.01)。ROC曲线显示DAO和D-乳酸曲线下的面积分别为0.925(95%CI 0.876~0.922;P<0.01)和0.841(95%CI 0.775~0.907;P<0.01)。DAO和D-乳酸的截断值分别为2152U/L和48.16U/L,敏感性和特异性分别为0.923、0.863和0.91、0.712。结论早期血清DAO和D-乳酸水平与AGI等级呈正相关,对AGI严重程度的评估有一定的临床意义。Objeetive To assess if the levels of early senum diamine oxidase(DAO)and D-lactic acid can reflect the severity of acute gastrointestinal injury(AGI)and obtain the best cut-of value.Methods A total of 151 patients with AGI admitted from April 2016 to April 2019 were enrolled into our study.According to AGI grade,the patients were divided to four groups:gradeⅠgroup,gradeⅡgroup,gradeⅢgroup and gradeⅣgroup.Serum DAO activity and D-lactic acid level were completed within 12hours.Results The DAO and D-lactic acid levels of the four groups were(3.35±2.03),(25.83±13.76)U/L(gradeⅠgroup),(14.92±17.33),(47.71±19.96)U/L(gradeⅡgroup)。(30.22±11.25),(59.23±14.08)U/L(gradeⅡgroup),(53.84±24.54),(68.09±12.85)U/L(gradeⅣgroup).There were stistically signific ant differences between the two groups at random(P<0.05).Pearson analysis showed that the correlation cofficient r was 0.741(95%CI 0.672~0.825,P<0.01)and 0.681(95%CI 0.589~0.758,P<0.01).ROC curve showed that the areas under the curve of D-lactic acid and DAO were 0.925(95%CI 0.876~0.922;P<0.01)and 0.841(95%CI 0.775~0.907,P<0.01),respectively.The cut-off values of DAO and D-lactie acid were respectively 21.52U/L and 48.16U/L.The sensitivity and specificity were 0.923 vs 0.863(DAO)and0.91 vs 0.712(D-lactic acid).Conclusion Early serum DAO and d-lactic acid levels are positively correlated with AGI grades,which is useful for predicting AGI severity.
