股骨头置换和锁定钢板内固定治疗高龄转子间骨折的疗效分析

摘　　要：目的比较股骨头置换和锁定钢板内固定治疗股骨粗隆间骨折的疗效方法纳入高龄股骨粗隆间骨折患者80例,按照手术方式分为观察组(股骨头置换,42例)和对照组(锁定钢板内固定,38例)评估手术时间、术中失血量、切口长度、住院时间、卧床时间、疼痛VAS评分和髋关节Harris功能情况结果观察组手术时间、术中失血量、切口长度高于对照组,差异有统计学意义(P<0.05)e观察组卧床时间明显较短,组间差异有统计学意义(P<0.05),随访3个月时,观察组VAS评分、Harris评分明显优于对照组,差异有统计学意义(P<0.05),随访2年时,两组患者VAS、Harris评分比较,差异无统计学意义(P>0.05)观察组并发症发生率9.5%,低于对照组13.1%,组间比较差异无统计学意义(P>0.05)结论股骨头置换治疗高龄股骨粗隆间骨折术中创伤较锁定钢板内固定大,但是卧床时间短,可早期下地行功能锻炼,利于患者早期融入正常生活,值得临床推广。Objeetive To explore the clinical efecet dfference of femoral intetrochanterie fracture treated by bipolar femoral head replacement and lock plate internal fixation.Method Patients with femoral intertrochantenic fracture were included in our study.All the patients were asigned into two groups(42 cases in experiment group and 38 cases in control group),which were receive bipolar femoral head replacement and lock plate intemal fixation.Some parameters like surgical time,intraopenative blood loss,impatient time,bed time,VAS score and Harris score were evaluated.Results For the surgical time,intraoperaive blood loss and incsion length aspect,the value of experimental group were higher than control group,there was signifcant difference between them(P<0.05).The bed time of experimental group was lower than control group,there was significant difference between them(P<0.05).The VAS score and Harris score of paients in 3 months had advantage than paients in control group.But,for complicacion rate and VAS score and Harris score in 2year,there was no sgnificant diference between two groups.Conclusion Bipolar femoral head replcement has disadvantage of intaoperative invasions than lock plate intemal fixation,but it alow patients free fiunctional excises,which has better clinical efect.
