摘 要:目的研究Vash1、Ki67、CD34在声带白斑组织中的表达情况,探讨其对声带白斑的诊断和预后的意义。方法2018年1月~2019年12月在黑龙江省医院耳鼻喉科住院接受支撑喉镜下手术治疗的声带白斑、声带息肉确诊病例共100例,其中声带白斑和声带息肉各50例,并对其进行随访。应用免疫组织化学染色法,检测Vash1、Ki67、CD34在声带白斑和息肉组织中的表达情况,比较不同病变中的表达差异,同时比较三者在声带白斑复发组与未复发组、癌变组与未癌变组中的表达差异。结果声带白斑组和声带息肉组间Vash1的表达比较,差异无统计学意义(χ^2=0.932,P=0.334),Ki67和CD34的表达比较,差异有统计学意义(Ki67:χ^2=8.208,P=0.004;CD34:P=0.000)。声带白斑非异型增生组和异型增生组间Vash1的表达比较,差异无统计学意义(χ^2=0.709,P=0.400),Ki67和CD34的表达比较,差异有统计学意义(Ki67:χ^2=4.303,P=0.038;CD34:P=0000)。Ki67与CD34在声带白斑组织中的表达呈正相关(r=0.526,P=0.000),Vash1与Ki67的表达呈正相关(r=0.378,P=0.007),Vash1与CD34的表达不相关(r=0.259,P=0.069)。在声带白斑复发组和未复发组间,Vash1的表达比较,差异无统计学意义(P=0.075),Ki67和CD34的表达比较,差异有统计学意义(Ki67:P=0.000;CD34:P=0.007),Vash1和Ki67在癌变组和未癌变间的表达比较,差异有统计学意义(Vash1:P=0.019;Ki67:P=0.023),CD34在两组间的表达比较,差异无统计学意义(P=0.085)。与声带息肉组织比较,声带白斑组织中Ki67、CD34呈高表达,病理表现为异型增生的白斑病变较非异型增生病变,Ki67、CD34呈高表达,Vash1在不同病变中的表达比较,差异无统计学意义。结论Ki67和CD34可能是声带白斑复发的预测指标,Vash1和Ki67可能是声带白斑癌变的预测指标。Objective To study the expressions and to investigate the significance of diagnosis and prognosis of Vash1,Ki67 and CD34 in the tissues of vocal cord leukoplakia.MethodsThis study included a total of 100 confirmed cases of vocal cord leukoplakia and vocal cord polyp,including 50 cases of vocal cord leukoplakia and 50 cases of vocal cord polyp,removed under laryngoscope from January 2018 to December 2019,in Heilongjiang Provincial Hospital,the Department of Otolaryngology,which were clinical followed up.The expressions of Vash1、Ki67、CD34 in vocal cord leukoplakia and polyp tissues were detected by immunohistochemical staining,whose expression differences were compared in different lesions.At the same time,the expression differences of the three proteins between recurrence group and non-recurrence group,canceration group and non-canceration group were compared.ResultsThere were no significant differences in the expression of Vash1 between vocal cord leukoplakia group and vocal cord polyp group(χ^2=0.932,P=0.334),but the expression differences of Ki67 and CD34 showed significances statistically(Ki67:χ^2=8.208,P=0.004;CD34:P=0.000).The expression differences of Vash1 between the non-hyperplasia group and the dysplasia group in the vocal cord leukoplakia group did not show significant differences statistically(χ^2=0.709,P=0.400),but the expressions of Ki67 and CD34 was statistically different(Ki67:χ^2=4.303,P=0.038;CD34:P=0.000).There was a positive correlation between the expressions of Ki67 and CD34 in vocal cord leukoplakia(r=0.526,P=0.000),so did the expressions of Vash1 and Ki67(r=0.378,P=0.007).But the expressions of Vash1 and CD34 had no correlation statistically(r=0.259,P=0.069).The expression differences of Vash1 was not statistically significant between recurrence group and non-recurrence group in vocal cord leukoplakia(P=0.075),but the expression differences of Ki67 and CD34 showed statistically significance(Ki67:P=0.000;CD34:P=0.007).The expression differences of Vash1 and Ki67 was statistically si
