摘 要:目的探讨miR-146a在脓毒症相关急性肺损伤患者中的表达及早期诊断价值。方法收集2018年6月至2019年6月入住我院ICU脓毒症患者60例为研究对象,其中按临床特征分为脓毒症组(30例)和脓毒症急性肺损伤组(30例),同时收集于本院体检中心体检的健康者20例作为对照研究。收集患者入院24h临床资料并采集血液样本检测超敏C反应蛋白并分离外周血单核细胞提取mRNA,采用荧光定量PCR法检测各组患者外周血单核细胞中miR-146a的表达情况,并分析评价miR-146a在脓毒症急性肺损伤中的诊断价值。结果脓毒症急性肺损伤组患者与脓毒症组患者年龄、性别等一般资料均无统计学差异;脓毒症急性肺损伤患者APACHEⅡ和CRP均高于脓毒症组患者且差异具有统计学意义,P<0.05;脓毒症急性肺损伤组患者和脓毒症组患者的外周血miR-146a表达量均高于健康对照组,其中脓毒症急性肺损伤组更高,差异具有统计学意义,P<0.05;ROC曲线分析显示,miR-146a曲线下面积和灵敏度均高于APACHEⅡ和CRP,特异度高于APACHEⅡ略低于CRP。结论 miR-146a在脓毒症急性肺损伤患者中高表达,且具有作为脓毒症急性肺损伤早期诊断标志物的可能性。Objective To explore the expression and early diagnosis value of miR-146 a in patients with sepsis-related acute lung injury.Methods Collect 60 patients with sepsis who were admitted to our hospital from June 2018 to June 2019,divided into sepsis group(30 cases)and sepsis acute lung injury group(30 cases)According to clinical characteristics.At the same time,collected the healthy people who were in the medical examination center of our hospital as for the control study.Collect the 24-hour clinical data of patients admitted and collect blood samples to detect hypersensitive C-reactive protein and the expression level of miR-146 a in peripheral blood mononuclear cell by RTPCR.Analysis and evaluation of the diagnostic value of miR-146 a in acute lung injury caused by sepsis.Results There was no statistically significant difference in age,sex and other general data between patients with sepsis acute lung injury and patients with sepsis.APACHEⅡ and CRP in patients with acute lung injury in sepsis were higher than those in sepsis group and the difference was statistically significant,P< 0.05.The expression of miR-146 a in peripheral blood of patients with sepsis acute lung injury and patients with sepsis was higher than that of healthy controls,especially the sepsis acute lung injury patients and the difference was statistically significant,P<0.05.ROC curve analysis shows that the area under the miR-146 a curve is 0.846(95% CI:0.765~0.927),the sensitivity is 84.38%,and the specificity is 73.58%.Conclusions miR-146 a is highly expressed in patients with acute lung injury in sepsis,and has the possibility of being a marker for early diagnosis of acute lung injury in sepsis.
