膝关节镜微创治疗股骨远端骨折术后继发创伤性膝关节炎的效果研究

摘　　要：目的探讨膝关节镜微创治疗股骨远端骨折术后继发创伤性膝关节炎的临床效果。方法86例股骨远端骨折术后继发创伤性膝关节炎患者,随机分为观察组和对照组,各43例。观察组采用微创关节镜下给予氨基酸葡萄糖注射治疗,对照组采用中药外敷及针灸治疗。观察比较两组患者的治疗效果及治疗前后膝关节美国特种外科医院(HSS)评分。结果治疗前,两组HSS评分比较,差异无统计学意义(P>0.05);治疗后,两组HSS评分均高于治疗前,且观察组HSS评分(89.26±6.34)分高于对照组的(72.15±3.34)分,差异均有统计学意义(P<0.05)。观察组总有效率为93.02%,高于对照组的69.77%,差异有统计学意义(P<0.05)。结论采用膝关节镜微创治疗股骨远端骨折术后继发创伤性膝关节炎,其效果显著,可有效改善膝关节功能,值得临床推广使用。Objective To discuss the clinical effect of minimally invasive treatment of secondary knee arthritis after distal femoral fracture.Methods A total of 86 patients with secondary knee arthritis after distal femoral fracture were randomly divided into observation group and control group,with 43 cases in each group.The observation group was treated with glucosamine injection under minimally invasive arthroscopy,while the control group was treated with external application of traditional Chinese medicine and acupuncture.The therapeutic effect and hospital for special surgery(HSS)score before and after treatment were compared between the two groups.Results Before treatment,there was no statistically significant difference in HSS score between the two groups(P>0.05).After treatment,HSS score of the two groups were higher than those before treatment,and HSS score(89.26±6.34)points of the observation group was higher than(72.15±3.34)points of the control group,and the difference was statistically significant(P<0.05).The total effective rate of the observation group was 93.02%,which was higher than 69.77%of the control group,and the difference was statistically significant(P<0.05).Conclusion Minimally invasive treatment is effective in secondary knee arthritis after distal femoral fracture,and it can effectively improve knee function.It is worthy of promotion and application.
