摘　　要：目的观察阴阳九针结合董氏奇穴针刺对脾胃气虚型神经性耳鸣患者耳鸣响度及中枢神经递质血清5-羟色胺(5-HT)、γ-氨基丁酸(GABA)含量的影响。方法将76例脾胃气虚型神经性耳鸣患者按照随机数字表法分为2组,在常规对症治疗基础上,对照组38例加常规取穴针刺疗法,治疗组38例加阴阳九针结合董氏奇穴针刺治疗,2组均隔日1次,10次为1个疗程,共治疗1个疗程。比较2组疗效,比较2组治疗前后耳鸣响度、中医证候评分、耳鸣残疾量表(THI)评分、医院焦虑抑郁量表(HADS)评分、匹兹堡睡眠质量指数(PSQI)评分及血清5-HT、GABA水平,并比较2组安全性。结果治疗后治疗组临床疗效优于对照组(P<0.05)。治疗后2组耳鸣响度,中医证候各单项评分和总分,THI各项评分及总分,HADS评分、PSQI评分均较本组治疗后降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后2组血清5-HT水平均较本组治疗前降低(P<0.05),GABA水平升高(P<0.05),且治疗组5-HT水平低于对照组(P<0.05),GABA水平高于对照组(P<0.05)。2组安全性评价比较差异无统计学意义(P>0.05)。结论阴阳九针结合董氏奇穴针刺治疗脾胃气虚型神经性耳鸣能够显著降低患者耳鸣响度和耳鸣严重程度,其作用机制可能与调节中枢神经递质5-HT、GABA水平有关。Objective To explore the effect of YinYang nine needle techniques combined with Dong's extraordinary acupoints on tinnitus volume and content of central neurotransmitters including serum 5-Hydroxytryptamine(5-HT)andγ-aminobutyric acid(GABA)in patients with nervous tinnitus of qi deficiency of spleen and stomach type.Methods 76 patients with nervous tinnitus of qi deficiency of spleen stomach type were divided into the treatment group(n=38)and the control group(n=38)according to random number table.On the basis of conventional symptomatic treatment,the control group was administered with conventional acupuncture therapy while the treatment group with YinYang nine needle techniques combined with Dong's extraordinary acupoints.And there were a total of two treatment courses(10 times per course).The scores of tinnitus volume,traditional Chinese medicine(TCM)syndrome,Tinnitus Handicap Inventory(THI),Hospital Anxiety and Depression Scale(HADS),and Pittsburgh Sleep Quality Index(PSQI)and the levels of serum 5-HT and GABA were compared between the two groups before and after treatment.Besides,the clinical therapeutic effect and safety in the two groups were evaluated and compared.Results The clinical therapeutic effect in the treatment group was better than that of control group(P<0.05).After treatment,the tinnitus volume score,single TCM syndrome and THI score and their total scores,HADS score and PSQI score of the two groups were reduced(P<0.05).And those scores in the treatment were lower than those in the control group(P<0.05).After treatment,the level of serum 5-HT in each group was decreased(P<0.05)but GABA increased(P<0.05)in comparison to the results before treatment.Meanwhile,5-HT in the treatment group was lower than that of control group(P<0.05)but GABA was higher(P<0.05).Additionally,there was no statistical significance about the safety evaluation comparison between the two groups(P>0.05).Conclusion YinYang nine needle techniques combined with Dong's extraordinary acupoints can significantly lower the tinni
