四联疗法序贯半夏泻心汤治疗青年人Hp感染性CAG临床观察

摘　　要：目的观察四联疗法序贯半夏泻心汤加减治疗青年人幽门螺杆菌(Helicobacter pylori,Hp)感染性慢性萎缩性胃炎(Chronic atrophic gastritis,CAG)的临床疗效。方法将68例Hp感染性CAG青年患者随机分为2组,治疗组和对照组均予铋剂四联疗法口服,复查Hp转阴后,治疗组34例序贯半夏泻心汤加减口服,对照组34例序贯叶酸片和替普瑞酮胶囊口服。观察2组治疗前后的中医症状积分变化、临床疗效及胃镜病理组织学疗效。结果2组患者治疗后中医证候积分包括主症以及次症积分和总积分比治疗前均下降(P<0.05),其中,治疗组次症以及总积分比对照组更低(P<0.05);治疗后,治疗组的临床总有效率为91.18%,对照组为70.59%,2组疗效比较差异具有统计学意义(P<0.05);治疗后,治疗组的胃镜病理组织学疗效总有效率为91.18%,对照组为79.41%,2组胃镜病理组织学疗效相比差异具有统计学意义(P<0.05)。结论四联疗法序贯半夏泻心汤加减治疗有利于改善青年人Hp感染性CAG患者的临床症状,提高生活质量,临床疗效显著,同时对萎缩或肠上皮化生有改善作用。Objective To observe the clinical effect of Quadruple therapy sequential Banxia Xiexin decoction in the treatment of Helicobacter pylori infectious chronic atrophic gastritis in young people. Methods 68 young patients with Helicobacter pylori infectious chronic atrophic gastritis were randomly divided into two groups. Both the treatment group and the control group were given bismuth quadruple therapy orally.After Hp turned negative,34 cases in the treatment group were treated with Banxia Xiexin decoction and 34 cases in the control group were treated with folic acid tablets and Teprenone capsule. The changes of TCM symptom score,clinical effect and gastroscope histopathological effect of the two groups before and after treatment were observed. Results After treatment,TCM syndrome score including main symptom score,secondary symptom score and total score of the two groups were significantly lower than before treatment( P < 0. 05). The scores of secondary symptoms and total scores in the treatment group were lower than those in the control group( P < 0. 05). After treatment,the total clinical effective rate of the treatment group was 91. 18%,and that of the control group was 70. 59%. The difference between the two groups was statistically significant( P < 0. 05). After treatment,the total effective rate of pathological tissue of gastroscope was 91. 18% in the treatment group and 79. 41% in the control group. The difference between the two groups was statistically significant( P < 0. 05).Conclusion Quadruple therapy sequential Banxia Xiexin decoction is beneficial to improve the clinical symptoms,quality of life and clinical effect of young patients with Helicobacter pylori infectious chronic atrophic gastritis. Moreover,it can improve atrophy or intestinal metaplasia.
