调元散加减联合维生素D对心脾两虚型脑瘫儿童疗效及血清IGF-1和IFN-γ水平的影响

摘　　要：目的:探讨调元散加减联合维生素D治疗心脾两虚型儿童脑瘫的疗效及对血清胰岛素样生长因子(IGF)-1和干扰素(IFN)-γ水平的影响。方法:将痉挛型脑瘫患儿260例按随机数字表法分为对照组和观察组各130例,2组患儿均给予常规康复训练,对照组加用双鲸维生素D滴剂,观察组在对照组治疗基础上用调元散加减治疗。连续治疗3个月后,比较2组患儿精细运动功能测试(FMFM)、粗大运动功能测试(GMFM)、Peabody精细运动发育量表(PDMS FM)、下肢肌力Ashworth量表评分、疗效以及血清IGF-1和IFN-γ水平。结果:治疗3个月后观察组FMFM量表(B区、C区、D区、E区)、GMFM量表(A区、B区、C区、E区、D区)、PDMS FM评分明显高于对照组,Ashworth量表评分明显低于对照组;观察组总有效率、血清IGF-1水平显著高于对照组,IFN-γ水平明显低于对照组。结论:调元散加减联合维生素D可促进心脾两虚型脑瘫儿童运动功能和肌张力康复,提高疗效,可能与其调节血中IGF-1与IFN-γ水平有关。Objective:To investigate the effect of Tiaoyuan decoction combined with vitamin D on cerebral palsy children with syndrome of deficiency of heart and spleen and its effect on serum levels of insulin-like growth factor(IGF-1)and interferon(IFN)-γ in the patients.Method:260 cerebral palsy children were randomly divided into the control and the observation group with 130 cases in each group according to the number table method.Both groups were given with routine rehabilitation training.The control group was given routine training plus vitamin D drops.On the basis of the control group,the observation group received Tiaoyuan decoction.After 3 months'treatment,the Fine motor function assessment scale(FMFM),Gross motor function assessment scale(GMFM),Peabody fine motor development scale(PDMS FM),Ashworth scale scores,curative effect and serum IGF-1 and IFN-γ levels were compared between the two groups.Result:After treatment,scores of FMFM scale(area B,C,D,E),GMFM scale(area A,B,C,E,D)and PDMS FM in the observation group were significantly higher,while Ashworth scale score was significantly lower than those in the control group.Total effective rate in the observation group was significantly higher than the control group.Serum level of IGF-1 in the observation group was significantly higher,while IFN-γ was significantly lower than those in the control group.Conclusion:Tiaoyuan decoction combined with vitamin D can promote the rehabilitation of motor function and muscle tone in cerebral palsy children with syndrome of deficiency of heart and spleen,and improve the curative effect,which may be related to its regulation effect on serum IGF-1 and IFN-γ levels..
