夹层杯离心集菌涂片法检测痰液抗酸杆菌的临床应用

摘　　要：目的:探讨夹层杯离心集菌涂片法检测痰液抗酸杆菌的临床应用价值。方法:对852份肺结核患者的痰标本,分别采取夹层杯集菌离心涂片法,直接涂片法和改良罗氏培养法检测抗酸杆菌,对比分析3种方法的阳性检出率,比较3种方法对不同时段的痰标本的抗酸杆菌阳性检出率。结果:夹层杯法、直接涂片法和罗氏培养法检测852例痰标本的阳性率分别为31.1%、17.5%和32.6%,夹层杯法阳性率明显高于直接涂片法(P<0.05);与改良罗氏培养法相比无明显差异(P>0.05);并且3种检测方法均以晨痰的阳性检出率最高,夹层杯法对不同时间痰标本的检出率与改良罗氏培养法无统计学差异(P<0.05)。结论:夹层杯离心集菌涂片法的操作简单、省时且抗酸杆菌的阳性检出率高,与改良罗氏培养法相比有较好的符合率,在临床上值得推广应用,并且应强调收集晨痰送检。Objective:To explore the clinical application value of the sandwich cup centrifuge smear method in the detection of sputum acid-fast bacilli.Methods:The sputum samples of 852 patients with pulmonary tuberculosis were detected by sandwich cup centrifuge smear method,direct smear method and modified Roche culture method to detect the acid-fast bacilli.The positive detection rate of the three methods was compared.The positive detection rate of acid-fast bacilli in the sputum specimens of different time periods by the three methods were compared.Results:The positive rates of 852 cases of morning sputum specimens by sandwich cup method,direct smear method and Roche culture method were 31.1%,17.5%and 32.6%,respectively.The positive rate of sandwich cup method was significantly higher than that of direct smear method(P<0.05),and compared with the modified Roche culture method,there was no significant difference(P>0.05).The positive detection rate of the morning sputum was the highest among the three methods,and the detection rate of sputum samples by sandwich cup method at different time was not significantly different from that by modified Roche culture method(P<0.05).Conclusion:The sandwich cup centrifuge smear method has the advantages of simple operation,time-saving and high positive detection rate of acid-fast bacilli,which is in good agreement with the modified Roche culture method,and is worthy of popularization and application in clinical practice,and the collection of morning sputum for examination should be emphasized.
