摘　　要：目的了解区域医联体模式下护士实施医院-社区一体化延续护理(transitional care model,TCM)的促进因素,为推进TCM提供理论依据和实践参考。方法自拟调查问卷,对292名东莞长安区域医联体内护士进行问卷调查,主要内容包括基本情况、TCM知晓、促进因素及对策4个维度。结果区域医联体模式下护士实施延续护理的促进因素排前3位是和谐医患关系、科学的考核机制和设立专职部门或人员,认同率分别为93.49%、88.36%、87.67%。不同级别的医院护士对和谐医患关系、科学的考核机制、设立专职部门或人员、TCM知晓高及规范双向转诊5个条目的认同率差异均有统计学意义(P<0.05),进一步两两比较发现,二级医院或社区医疗机构护士对于5个条目的认同率显著高于一级医院护士。结论区域医联体模式下护士实施医院-社区一体化TCM促进因素影响力各有侧重,当前应抓住重点促进因素,加快构建和谐医患关系,设计科学考核体系,完善保障措施。Objective This study was aimed to explore promoting factors of nurses from a regional medical consortium in chang’an town of Dongguan implementing hospital-community integrative collaborative transitional care model(TCM),and to provide theoretical basis and practice reference for deepen TCM implementation.Methods A total of 292 nurses from a regional medical consortium were investigated by self-made questionnaire with four dimensions including general sociodemographic characteristics,awareness level of TCM,promoting factors and countermeasures.Results The promotion factors to implementing TCM with top three approval ratings were harmonious doctor-patient relationship(93.49%),scientific evaluation system(88.36%)and setting up the full-time department or personnel(87.67%).There were significant differences in the approval ratings on harmonious doctor-patient relationship of nurses from different degree hospitals(P<0.05),on scientific evaluation system,on setting up the full-time department or personnel,on high awareness level of TCM and on regulating dual referral.Further pairwise comparison methods used the results showed that approval ratings on all above five items of nurses from secondary hospitals or community health service center were significant higher than from primary hospitals.Concusion The promoting factors of nurses from a regional medical consortium in chang’an town of Dongguan implementing TCM have own emphasis.Currently it is necessary to seize key factors to promote,speed up the construction of a harmonious doctor-patient relationship,design scientific evaluation system and perfect the safeguard measures.
