摘  要:目的研究探讨磁共振弥散张量成像(diffusion tensor imaging,DTI)对弥漫性轴索损伤患者(diffuse axonal injury,DAI)的早期诊断和预后评估的价值。方法随机选取2016年1月—2020年2月期间,在本院住院的40例DAI患者为研究对象(DAI组),回顾性分析研究对象的临床资料,选取40例健康志愿者为对照组,分析DAI组的影像特点,对比两组胼胝体压部、大脑额叶白质、内囊、丘脑等感兴趣区(region of interest,ROI)各向异性分数(fractional anisotropy,FA)值和表观弥散系数(apparent diffusion coefficient,ADC)值的特点。结果DAI组的胼胝体压部、大脑额叶白质、内囊、丘脑等感兴趣区的ADC值和FA值均低于对照组。结论磁共振弥散张量成像能够敏感反应脑白质损伤情况,对弥漫性轴索损伤的早期诊断价值较高,并可以动态观察弥漫性轴索损伤变化,对其预后评估意义较大。Objective To investigate the value of diffusion tensor imaging(DTI)on the early diagnosis and prognosis of diffuse axonal injury(DAI).Methods From January 2016 to February 2020,40 DAI patients in the hospital were selected as subjects(DAI group).The clinical data of the subjects were analyzed.Forty healthy volunteers were selected as the control group.The image characteristics of DAI group were analyzed.Compare the characteristics of the two groups of corpus callosum,cerebral frontal lobes,inner cysts,thalamus,etc.,and the characteristics of the values of anisotropy(FA)and apparent diffusion coefficient(ADC).Results The ADC values and FA values of the corpus callosum,the frontal lobes,the inner cysts and thalamus of the DAI group were lower than those in the control group.Conclusion Magnetic resonance diffusion tensor imaging can sensitively reflect brain white matter damage,and is of high value in the early diagnosis of diffuse axonal injury.It can also dynamically observe the changes of diffuse axonal injury,which is of greater significance for its prognostic evaluation.
