脑室引流结合尿激酶治疗全脑室出血62例分析

摘　　要：目的探讨尿激酶溶解结合脑室引流、冲洗治疗全脑室出血的有效性和安全性。方法选取2011年1月至2019年6月住院治疗的112例高血压脑出血破入全脑室的患者作为研究对象,根据治疗方法的不同分为A组(n=62)与B组(n=50)。A组采用尿激酶溶解结合脑室引流、冲洗治疗,B组拒绝手术采用内科保守治疗,比较两组治疗结果。结果 A组:良好和中残34例(54.8%)、重残14例(22.6%)、植物生存10例(16.1%)、死亡4例(6.5%);有效58例,无效4例,有效率93.5%。B组:中残2例(4.0%)、重残2例(4.0%)、植物生存2例(4.0%)、死亡44例(88.0%);有效6例,无效44例,有效率12.0%。两组效果及有效、无效情况比较差异有统计学意义(P<0.001)。结论尿激酶溶解结合脑室引流、冲洗治疗在高血压脑出血破入全脑室患者中效果显著,极大降低病死率,提高生存质量。Objective To explore the efficacy and safety of urokinase dissolution combined with ventricular drainage and washing in the treatment of total ventricular hemorrhage.Methods 112 patients with hypertensive cerebral hemorrhage broken into the whole ventricles of the brain who were hospitalized from January 2011 to June 2019 were selected as the study subjects,and divided into group A(n=62)and group B(n=50)according to different treatment methods,group A was treated with urokinase dissolution combined with ventricular drainage treatment group,and group B refused to surgery,was treated with internal medicine conservative treatment group.The treatment results of the two groups were statistically analyzed and compared.Results In group A,34 cases(54.8%)were good and moderate disability,14 cases(22.6%)were severe disability,10 cases(16.1%)were persistent vegetative state,4 cases(6.5%)were death;58 cases were effective and 4 cases were ineffective,Effective rate(93.5%).In group B,2 cases(4.0%)were moderate disability,2 cases(4.0%)were severe disability,2 cases(4.0%)were persistent vegetative state,44 cases(88.0%)were death;6 cases were effective and 44 cases were ineffective,effective rate(12.0%).These was significantly difference between group A and group B by chi square test(P<0.001).Conclusion Urokinase dissolution combined with ventricular drainage and washing in the treatment of hypertensive cerebral hemorrhage and broke into the whole ventricle has a significant effect,greatly reducing the mortality and improving the quality of life.
