摘　　要：目的探讨改良Scarf截骨锁定钢板固定治疗老年女性中重度拇外翻畸形的临床疗效。方法回顾性分析2016年4月至2018年10月收治的采用改良Scarf截骨锁定钢板固定治疗的26例中重度老年女性拇外翻患者的临床资料,比较手术前后拇外翻角(hafiux valgus angle,HVA),第1、2跖骨间角(intermetatarsal angle,IMA),并采用美国足踝外科协会第一跖趾关节评分(AOFAS)评价临床疗效。结果 26例患者均获随访,时间12~16个月,平均(14.8±4.2)个月。手术切口均未发生感染、无皮肤坏死及神经损伤发生。术前HVA、IMA高于术后6个月,差异有统计学意义(P<0.05)。术前AOFAS评分低于术后6个月,差异有统计学意义(P<0.05)。结论改良Scarf截骨锁定钢板固定治疗老年女性中重度拇外翻畸形,可有效纠正拇外翻畸形,牢固固定,改善患者临床症状。Objective To explore the clinical curative of modified Scarf osteotomy and locking steel plate fixing in treatment of moderate to severe elderly women hallux valgus.Methods The clinical data of 26 elderly women patients with moderate to severe hallux valgus treated by modified Scarf osteotomy and locking steel plate fixing from April 2016 to October 2018 were retrospectively analyzed.Hallux valgus angle(HVA)and intermetatarsal angle(IMA)between the first and the secondmetatarsal were examined before and after operation.AOFAS score was applied to evaluate clinical effects.Results A follow-up lasted 12-16 months[mean(14.8±4.2)months]in 26 patients.There were no nonunion,skin necrosis and nerve damage.Preoperative HVA and IMA were higher than those after operation at 6 months,and had statistical difference before and after operation(P<0.05).A0FAS score before operation was lower than that after operation at 6 months(P<0.05).Conclusion Modified Scarf osteotomy and locking steel plate fixing can correct moderate to severe hallux valgus,firmly fixed,and improve the clinical symptoms effectively.
