二仙消瘿汤联合131I治疗桥本甲状腺炎对TgAb、TPOAb浓度的影响

摘　　要：目的:分析二仙消瘿汤联合131I治疗桥本甲状腺炎(HT)对甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)浓度的影响。方法:选取2018年2月~2020年1月我院收治的100例HT患者为研究对象,采用随机数字表法将其分为观察组、对照组各50例,对照组予以131I及常规治疗,观察组在此基础上予以二仙消瘿汤治疗,比较两组临床疗效、治疗前后相关抗体(TgAb、TPOAb)、甲状腺大小,比较两组甲状腺激素[游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)]、炎性因子[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)]水平变化,并记录不良反应发生情况。结果:观察组治疗有效率高于对照组(P<0.05);观察组治疗后TgAb、TPOAb水平低于对照组,而甲状腺大小小于对照组(P<0.05);观察组治疗后血清FT3、FT4高于对照组,而TSH水平低于对照组(P<0.05);观察组治疗后血清CRP、TNF-α、IL-6水平低于对照组(P<0.05);两组均未出现明显不良反应。结论:二仙消瘿汤联合131I治疗HT疗效较好,可明显改善患者TgAb、TPOAb浓度及甲状腺激素水平,缩小甲状腺大小,降低炎性因子水平,安全性可靠,值得在临床推广实践。Objective:To analyze the effect of using Erxian Xiaoying Decoction combined with 131I on the concentration of thyroglobulin antibody(TGAb)and thyroperoxidase antibody(TPOAb)in the treatment of Hashimoto’s thyroiditis(HT).Methods:100 patients with HT admitted to our hospital from February 2018 to January 2020 were selected as the study objects.They were divided into observation group and controlled group with 50 cases each by random number table method.The controlled group was treated with 131I and routine treatment,and the observation group was treated with Erxian Xiaoying Decoction on this basis.The clinical effects,the relevant antibodies(TGAb,TPOAb)and thyroid size before and after treatment were compared.The levels of thyroid hormones[Free triiodothyronine(FT3),free thyroxine(FT4),thyrotropin(TSH)],inflammatory factors[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)]were compared,and adverse reactions were recorded.Results:The effective rate of treatment in the observation group was higher than that in the controlled group(P<0.05).The level of TGAb and TPOAb in the observation group was lower than that in the controlled group,while the size of thyroid gland was smaller than that in the controlled group(P<0.05).The level of serum FT3 and FT4 in the observation group was higher than that in the controlled group,while the level of TSH was lower than that in the controlled group(P<0.05).The level of serum CRP,TNF-αand IL-6 in the observation group was lower than that in the controlled group(P<0.05).Neither group had obvious adverse reactions occurred.Conclusion:Erxian Xiaoying Decoction combined with 131I is effective in the treatment of HT.It can obviously improve the concentration of TGAb,TPOAb and the level of thyroid hormones,reduce the size of thyroid gland,reduce the level of inflammatory factors.It is safe and reliable.It is worth popularizing in clinical practice.
