三藤四物汤加减结合体外冲击波治疗骨盆旋移综合征临床疗效观察

摘　　要：目的:观察三藤四物汤加减结合体外冲击波治疗骨盆旋移综合征的临床疗效。方法:选取我院骨盆旋移综合征患者68例,随机分为对照组(n=34)和治疗组(n=34)。对照组采用正骨复位手法,锻炼骨盆肌群肌力;治疗组在此基础上联合体外冲击波治疗及三藤四物汤。比较两组间临床疗效,治疗前后进行影像学检测,记录髋骨宽度差、闭孔宽度差、股骨颈长度差、髂嵴高度差、腰骶角等数值,采血测定血清肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、白介素-6(Interleukins-6,IL-6)数值,记录不良反应事件。结果:对照组总有效率76.47%低于治疗组94.12%,有统计学差异(P<0.05)。治疗后对照组和治疗组髋骨宽度差分别为(5.67±2.12)、(3.82±2.01),闭孔宽度差分别为(2.69±1.16)、(1.72±1.20),股骨颈长度差分别为(1.96±1.13)、(1.18±1.02),髂嵴高度差分别为(4.33±2.26)、(2.75±2.18),血清TNF-α分别为(3.20±1.21)ng/mL、(2.19±1.14)ng/mL,血清IL-6分别为(10.13±2.24)pg/mL、(7.94±2.10)pg/mL,对照组上述指标与治疗组比较,有统计学差异(P<0.05)。两组间不良率比较,无统计学差异(P>0.05)。结论:三藤四物汤加减结合体外冲击波治疗骨盆旋移综合征的临床疗效确切,改善影像学指标,降低炎症因子,且不加重不良反应,值得广泛推广。Objective:To explore the clinical effect of applying Modified Santeng Siwu Decoction combined with extracorporeal shock wave in the treatment of pelvic rotation syndrome.Methods:68 cases of pelvic rotation syndrome were randomly divided into controlled group and treatment group,34 cases respectively.The controlled group received orthopedic reduction technique to exercise the muscle strength of pelvic muscle group,the treatment group received extracorporeal shock wave,and Modified Santeng Siwu Decoction.Before and after treatment,levels of hip width,obturator width,femoral neck length,iliac crest height and lumbosacral angle were recorded.The values of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and adverse events were measured.Results:The total effective rate of the controlled group(76.47%)was lower than the treatment group(94.12%),(P<0.05).After treatment,the difference of hip width were(5.67±2.12),(3.82±2.01),obturator width were(2.69±1.16),(1.72±1.20),femoral neck length were(1.96±1.13),(1.18±1.02),iliac crest height were(4.33±2.26),(2.75±2.18),serum TNF-αwere(3.20±1.21)ng/ml,(2.19±1.14)ng/ml,IL-6 were(10.13±2.24)pg/ml and(7.94±2.10)pg/ml,and all the indexes had significant differences(P<0.05).There was no significant difference between the two groups(P>0.05).Conclusion:The clinical effect of applying Modified Santeng Siwu Decoction combined with extracorporeal shock wave in the treatment of pelvic rotation syndrome is accurate,and it can improve imaging indexes,reduce inflammatory factors,and not aggravate adverse reactions,which is worth popularizing.
