摘 要:目的:探究不同口腔癌颈淋巴转移情况患者乏氧诱导因子(hypoxic induction factor,HIF-1)、白细胞介素-8(interleukin-8,IL-8)、Smad4表达的影响。方法:选取2015年1月~2018年12月接受治疗的54例原发性口腔癌患者,并同时从医院周边的3个居民区内采用随机分层抽样的方法抽取54名健康居民作为对照组。应用免疫组织化学方法分别检测54例口腔癌患者和健康居民口腔黏膜组织,采用Logistic回归分析方法比较HIF-1、IL-8、Smad4的表达情况与肿瘤发展情况之间的关系。结果:健康居民HIF-1、IL-8阳性表达情况显著低于口腔癌患者,Smad4阳性表达情况显著高于口腔癌患者(P<0.05);已发生转移口腔癌患者Smad4阳性率明显低于未发生转移者(P<0.05)。以口腔癌患者淋巴转移为因变量,以HIF-1、IL-8、Smad4阳性表达情况为自变量,拟合出二分类Logistic回归方程,各自变量对应变量影响大小依次为:Smad4(OR=3.896),淋巴转移(OR=3.353),IL-8(OR=2.858),HIF-1(OR=2.638)。结论:HIF-1、IL-8阳性表达的升高与Smad4阳性表达的降低可作为提示口腔癌颈淋巴转移的指标。Objective:To explore the influence of hypoxic induction factor(HIF-1),interleukin-8(IL-8) and Smad4 expression in patients with different cervical lymph node metastasis of oral cancer.Methods:From January 2015 to December 2018,all 54 patients with primary oral cancer and 54 healthy residents as controls were selected.Immunohistochemistry was used to detect the oral mucosal tissues of patients with oral cancer and healthy residents,and Logistic regression analysis was used to compare the expression of HIF-1,IL-8 and Smad4 with the tumor development of the patients.Results:The positive expression of HIF-1 and IL-8 in healthy residents was significantly lower than that in patients with oral cancer,and the positive expression of Smad4 in healthy residents was significantly higher(P<0.05).The positive rate of Smad4 in patients with metastatic oral cancer was significantly lower than that in patients without metastasis(P<0.05).The lymph node metastasis of oral cancer patients was the dependent variable,and the positive expressions of HIF-1,IL-8 and Smad4 were the independent variables.The binary logistic regression equation was fitted,and the influence of the corresponding variables was Smad4(OR=3.896),lymphatic metastasis(OR=3.353),IL-8(OR=2.858)and HIF-1(OR=2.638),respectively.Conclusion:The increase of hiF-1 and IL-8 positive expressions and the decrease of Smad4 positive expressions can be used as indicators of neck lymph node metastasis of oral cancer.
