腰硬联合麻醉与全身麻醉用于老年股骨胫骨骨折手术的临床观察

摘　　要：目的分析对老年股骨胫骨骨折手术患者实施腰硬联合麻醉及全身麻醉的临床效果。方法选取郓城县人民医院2018年2月至2019年2月82例老年股骨胫骨骨折手术患者作为研究对象,按照随机数字表法分为试验组与对照组,各41例,对照组患者采用全身麻醉,试验组患者采用腰硬联合麻醉,对比两组患者麻醉效果、麻醉后血液动力学指标变化情况及患者不良反应发生率。结果试验组患者麻醉效果、术后1 h麻醉后血液动力学指标变化均优于对照组,试验组患者不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论通过对老年股骨胫骨骨折手术患者实施腰硬联合麻醉的麻醉效果优于全身麻醉,腰硬联合麻醉药物剂量小、起效快,不良反应发生率低,且临床价值较高,值得广泛使用及推广。Objective To analyze the clinical effects of combined lumbar and general anesthesia in the surgery of elderly patients with femoral tibial fracture.Methods A total of 82 elderly patients with femoral and tibial fracture surgery from February 2018 to February 2019 in Yuncheng People's Hospital were selected as the research object,and they were divided into experimental group and control group using a random number table method,41 cases each group.Patients in the control group were given general anesthesia,and patients in the experimental group were given combined spinal-epidural anesthesia.The effects of anesthesia,changes in hemodynamic indicators after anesthesia and the incidence of adverse reactions in the two groups were compared.Results The effect of anesthesia and the changes of hemodynamics after operation in experimental group were better than those in control group and the incidence of adverse reactions in the esperimental group was lower than that in the control group,and there was on differences between the two groups,which was all statistically significant(P<0.05).Conclusion Through the elderly patients with femoral and tibial fracture surgery,the anesthesia effect of combined spinal and epidural anesthesia is better than general anesthesia,the dosage of combined spinal epidural anesthesia is small,the onset of effect is fast,the incidence of adverse reactions is low,and the clinical value is high,which is worthy of wide use and promotion.
