摘  要:目的探讨α-硫辛酸联合依帕司他对糖尿病周围神经病变神经传导的影响。方法选取2018年1月~2020年3月钦州市第一人民医院内分泌科住院的2型糖尿病合并周围神经病变患者80例,按照随机数字表法分为两组,每组各40例。对照组静脉滴注α-硫辛酸注射液,观察组在对照组基础上使用依帕司他胶囊,治疗1个疗程后,比较两组神经功能缺陷评分(NDS)变化情况;股动脉超声多普勒指标变化情况;统计两组治疗相关并发症情况;两组针对糖尿病周围神经病变干预的整体临床效果。结果干预后10d观察组NDS评分指标中踝反射、大拇指振动感觉、针刺感觉、温度感觉得分均明显低于对照组(P<0.05),观察组股动脉超声多普勒指标中V m和V s水平均显著大于对照组(P<0.05),P I指数小于对照组(P<0.05),观察组发生高血糖、低血糖的总比例显著低于对照组(P<0.05),观察组有效率显著高于对照组(P<0.05)。结论针对2型糖尿病周围神经病变患者,在应用α-硫辛酸基础上联合使用依帕司他,在一定程度上能改善外周神经功能,减少及延缓并发症发生,改善患者预后。Objective To investigate the effect ofα-lipoic acid combined with epalrestat on nerve conduction in diabetic peripheral neuropathy.Methods A total of 80 patients with type 2 diabetes mellitus complicated with peripheral neuropathy hospitalized in Department of Endocrinology of the First People's Hospital of Qinzhou from January 2018 to March 2020 were selected and divided into two groups according to random number table method,with 40 patients in each group.The control group receivedα-lipoic acid injection intravenously,and the observation group was administered epalrestat capsule on the basis of the control group.After a course of treatment,the changes of nerve deficiency scale(NDS)scores and the changes of ultrasonic Doppler indexes of femoral artery in the two groups were compared,and the treatmentrelated complications of the two groups were counted in addition to the overall clinical effect of the intervention on diabetic peripheral neuropathy in the two groups.Results At the 10th day of prognosis,among the NDS indexes,the scores of ankle reflex,thumb vibration feeling,acupuncture feeling and temperature feeling of the observation group were significantly lower than those of the control group(P<0.05).Among ultrasonic Doppler indexes of femoral artery,the levels of Vm and Vs in the observation group were both significantly higher than those in the control group(P<0.05),and the perfusion index was lower than that in the control group(P<0.05).The total proportions of hyperglycemia and hypoglycemia in the observation group were significantly lower than those in the control group(P<0.05),and the effective rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusion The combined use of epalrestat on the basis ofα-lipoic acid can improve the peripheral nerve functions,reduce and delay the complications and improve the prognosis of the patients with type 2 diabetes mellitus with peripheral neuropathy.
