氢吗啡酮复合右美托咪定对髋部骨折术后患者麻醉效果及情绪的影响

摘　　要：目的:浅析氢吗啡酮复合右美托咪定对髋部骨折术后对患者麻醉效果及情绪的影响。方法:纳入本单位于2018年1月至2019年12月间收治的68例髋部骨折手术患者作为研究样本,采用随机数字表法划分为观察组与对照组各34例,观察组术后镇痛泵配方为氢吗啡酮复合右美托咪定,对照组单独应用氢吗啡酮,分别记录2组患者术后1h、6h、24h的Ramsay镇静评分,SAS(焦虑自评量表)、心境量表(POMS)评分情况。结果:两组术后6h、24h镇静评分均低于术后1h,P<0.05;两组术后1h、6h、24h镇静评分比较无统计学意义,P>0.05。观察组术后1h、6h、24h疼痛评分均低于对照组,差异有统计学意义(P<0.05)。术前两组SAS评分、POMS评分结果比较无统计学意义,P>0.05;术后24h两组患者的SAS、POMS评分均低于术前;当观察组患者术后24h的SAS评分、POMS评分结果明显低于对照组,(t=7.271、3.711,P<0.05)。结论:髋部骨折中应用氢吗啡酮复合右美托咪定的镇静、镇痛效果良好,可有效改善患者不良情绪。Objective:To analyze the anesthesia effects and emotional changes with Hydromorphone and Dexmedetomidine after operation of hip fracture.Methods:68 patients of undergoing the operation of hip fracture from January 2018 to December 2019 in our hospital were chosen as research samples.According to random number table,patients were divided into observation group(34 cases)and control group(34 cases).The observation group took patient-controlled analgesia pump with Hydromorphone and Dexmedetomidine;the control group just took Hydromorphone.After 1h,6h and 24h of operation,Ramsay sedation scale,SAS(self-rating anxiety scale)and POMS(profile of mood states)scores were recorded.Results:After 6h and 24h of operation,sedation scores for two groups were lower than those after 1h of operation(P<0.05);after 1h,6h and 24h of operation,sedation scores between groups were not significantly different(P>0.05);after 1h,6h and 24h of operation,pain scores in the observation group were significantly lower than control group(P<0.05);before operation,SAS and POMS scores between groups were not significantly different(P>0.05);after 24h of operation,SAS and POMS scores for two groups were lower than before;after 24h of operation,SAS and POMS scores in the observation group were significantly lower than control group(t=7.271,3.711;P<0.05).Conclusion:After operation of hip fracture,Hydromorphone combined with Dexmedetomidine can increase the sedation and analgesia effects,and relieve the negative moods.
