摘　　要：目的观察成人两种乙型肝炎(乙肝)疫苗(Hepatitis B vaccine,HepB)—20μg重组酵母HepB(HepB-Y)和20μg重组CHO HepB(HepB-CHO)序贯免疫的免疫原性。方法在陕西省2个县招募乙肝五项血清标志物均阴性的26-45岁成人,随机分为四个组,按照0-1-6月免疫程序分别接种3剂HepB-Y、3剂HepB-CHO、2剂HepB-Y和1剂HepBCHO、2剂HepB-CHO和1剂HepB-Y,检测全程免疫后30-45d血清乙肝表面抗体(HBsAb),比较抗体阳转率和几何平均浓度(GMC)。结果四个组研究对象HepB全程免疫后HBsAb阳转率分别为93.62%(88/94)、93.48%(129/138)、99.00%(99/100)、94.79%(91/96)(χ^2=5.17,P=0.147),GMC(IU/L)分别为2039.74、3158.70、2672.09、3531.59(F=1.36,P=0.254)。结论成人序贯接种20μg HepB-Y和20μg HepB-CHO不降低HepB全程免疫的免疫原性。Objective To assess the immunogenicity of sequential vaccination with two hepatitis B vaccines(HepB)–a 20μg HepB made using a yeast substrate(HepB-Y)and a 20μg HepB made with CHO cells(HepB-CHO)in adults.Methods We recruited 26-45-year-old adults from two Shaanxi province counties who were negative to five serum markers of hepatitis virus.We divided them randomly into four groups-to receive either 3 doses of HepB-Y,3 doses of HepB-CHO,2 doses of HepB-Y followed by 1 dose of HepB-CHO,or 2 doses of HepB-CHO followed by 1 dose of HepB-Y as per the 0-1-6-month immunization schedule.We tested serum samples for hepatitis B surface antibody(HBs Ab)30-45 days after full-series vaccination to compare seroconversion rates and geometric mean concentrations(GMCs)of antibodies.Results Seroconversion rates of HBsAb for the four groups were 93.62%(88/94),93.48%(129/138),99.00%(99/100),and 94.79%(91/96)(χ^2=5.17,P=0.147);GMCs(IU/L)were 2039.74,3158.70,2672.09,and 3531.59(F=1.36,P=0.254),respectively.Conclusion Sequential vaccination with 20μg HepB-Y and 20μg HepB-CHO in adults did not reduce immunogenicity of hepatitis B vaccination that used the same vaccine for all three doses.
