腹腔镜疝修补术治疗腹股沟疝合并子宫圆韧带囊肿的临床体会

摘　　要：目的:探讨腹腔镜疝修补术治疗成年女性腹股沟疝合并子宫圆韧带囊肿的临床经验及疗效。方法:回顾分析2016年6月至2019年12月收治的6例成年女性腹股沟疝合并子宫圆韧带囊肿患者的临床资料,行经腹腹膜前疝修补术或全腹膜外疝修补术+子宫圆韧带囊肿切除术。结果:6例患者中2例行全腹膜外疝修补术,4例行经腹腹膜前疝修补术,均在腹腔镜下完成手术,无一例中转开放。术中均证实为腹股沟斜疝合并子宫圆韧带囊肿,无一例发现对侧隐匿疝。疝环直径平均(2.0±0.6)cm,手术时间平均(47±9)min。术后住院(2.5±0.5)d。术后随访(24±15)个月,无疝复发及囊肿复发、慢性疼痛、感染等并发症。结论:腹腔镜疝修补术治疗女性腹股沟疝合并子宫圆韧带囊肿是安全、可行的,疗效确切。Objective:To investigate the clinical experience and efficacy of laparoscopic repair of inguinal hernia with uterine round ligament cyst in adult female.Methods:A retrospective analysis was performed on clinical data of six adult women with inguinal hernia with uterine round ligament cyst from Jun.2016 to Sep.2019.All the patients underwent laparoscopic transabdominal preperitoneal or totally extraperitoneal prosthesis and uterine round ligament cyst resection.Results:All 6 patients laparoscopic surgery without conversion to open surgery.There were 2 cases of totally extraperitoneal prosthesis and 4 cases of transabdominal preperitoneal repair.All the 6 patients were intraoperatively diagnosed with indirect inguinal hernia combined with uterine round ligament cyst,and no contralateral concealed hernia was found.Hernia rings were(2.0±0.6)cm in diameter.The operation time was(47±9)min.The postoperative hospital stay was(2.5±0.5)d.The follow-up period was(24±15)months.No hernia and cyst recurrence,chronic pain or infection occurred.Conclusions:Laparoscopic inguinal hernia repair for adult female with inguinal hernia and uterine round ligament cyst is safe,feasible and effective.
